Note: This form must be submitted to Division HSSE Specialist & Corporate QHSSE Manager prior to pumping stage 1.

Inspection Items

HSE

  • Emergency contact numbers, ERP, GPS and directions posted in Data Van/Customer Trailer

  • Appropriate JSEAs discussed and signed

  • Sufficient absorbent materials on location to manage spills

  • LO/TO equipment available

  • Trip Hazards marked/eliminated

  • Ground rods in place and equipment connected properly (Data Van, Shower trailer, Fuel Trucks)

  • Line backers used to guard cables

  • Adequate room for egress between equipment

  • Wheels chocked on equipment with running road engines

  • Confined spaces identified with placards

  • MSDS available for every chemical on location

  • MSDS available in shower trailer for quick reference

  • Spill kits on location and available

  • First Aid kits inspected and stocked

Backside (Blender, C.A.S., Hydration, Sand)

  • Bridges used to keep hoses off sides of containment

  • Acid Lines marked with Pink Ribbon

  • Brine Lines marked with Orange Ribbon

  • Chemical hoses marked at every connection and coded properly (Primary= 1 stripe, Back up= 2 stripes)

  • Chemical hoses ran together to eliminate unnecessary trip hazards

  • Suction hoses (Red) used on suction side

  • Chemical hoses machine crimped or double banded

  • Boost hoses (blue) used on discharge side

  • Boost hoses restrained with whip check

  • Unused ports on blender/hydration capped (discharge)

  • Guards in place on Sand King stingers

  • Chemical Exclusion Zone set up

  • Respiratory Protection signs in place around sand area

  • Shower Trailer set up within 55ft of Chemical area and function tested

  • Water in shower trailer tested for Cl2 level

  • Hand Rails set up on each mover

Front Side (Pumps)

  • Pumps spotted with enough room for egress

  • Pumps spotted with enough room for work platforms to extend

  • Whip checks installed on boost hoses

  • Restraints used for all independent pumps and lines from front of missile forward

  • Iron certification within annual requirement

  • Iron banded within quarterly date

  • Swivel carriers available and used during rig up

  • Fire extinguishers inspected and placed on front of tractors

  • Lifting devices inspected and register up to date

  • Fall arrest devices inspected and register up to date

  • 75ft exclusion zone set up around pumping equipment and well head

  • Exclusion zone marked off at front of pumps

Wireline

  • Lifting equipment inspected and register up to date

  • Fall arrest devices inspected and register up to date

  • WL sheaves and chains inspected and register up to date

  • BOP function test

  • Hydraulic lines secured with whip checks

  • Back of perforator outside of exclusion zone

  • Catch pan under bottom sheave

  • Perforator grounded

  • Wheels chocked on perforator

Crane

  • Confirm anti two block test

  • Daily crane inspection forms available for review in crane cab

  • Max 30 days inspections in cranes (all others brought back to yard for record keeping)
