  • Do all guards and safety devices appear to be in place and functioning (to the best of your knowledge)? E.g. attachment locking pins, guards/shields, emergency stops/dead-man-switch, Operator Protective Structures (OPS) on mobile plant, safety belts, extraction systems, warning alarms etc.

  • Are plant/machinery items warranted, certified, tested etc. as required?

  • Is plant/machinery being used safely by competent operators?

  • Are workers maintaining a safe position and distance from mobile plant / raised booms?

  • Do lifting chains have current certification tags?

  • Do electrical items have current tags?

Excavations

EXCAVATIONS

  • Are excavations physically isolated when unattended? E.g. Covered or fenced (either around the excavation or the whole site)

  • Do excavations / trenches appear to be adequately battered, benched or shielded to prevent collapse?

  • Is there safe access into and out of excavations / trenches?

  • Are there adequate measures in place to manage water accumulation?

  • Are materials and plant kept back a safe distance from edges of excavations / trenches (minimum of 600mm)?

  • Have all underground services been identified, fully exposed by safe methods, and supported/protected where required?

  • Is work required in any excavation / trench that exceeds 1.5m deep?

  • Is the Worksafe notification for particular hazardous works available on site?

Hazardous Substances

HAZARDOUS SUBSTANCES

  • Are all hazardous substances listed in the inventory?

  • Is a current SDS available in OneDrive for all hazardous substances?

  • Are hazardous substances stored in the correct containers and clearly and accurately labelled?

  • Are hazardous substances stored safely? E.g. isolated from unauthorised access, properly contained to prevent them from being released accidentally into the environment, away from incompatibles or ignition sources etc.

  • Are workers aware of the risks posed by the hazardous substances they are working with, and the controls required? E.g. can they describe them to you?

  • Do workers have the necessary training and competency to handle hazardous substances as required?

  • Are workers using the correct PPE when working with hazardous substances?
