Safe Work Method Statements (SWMS) Safe work practices and Procedures

  • Involves the use of explosives; or

  • Is carried out on or near pressurised gas distribution mains or piping; or

  • Is carried out on or near chemical, fuel or refrigerant lines; or

  • Is carried out on or near energised electrical installations or services; or

  • Is carried out in an area that may have a contaminated or flammable atmosphere; or

  • Is carried out on, on or adjacent to a road, railway, shipping lane or other traffic corridor that is in use by traffic other than pedestrians; or

  • Is carried out in or near water or other liquid that involves a risk of drowning; or

  • Involves diving work; or

  • Other statements where the PC concludes an activity could result in death or bodily harm

Safe Work Procedures

  • The tenderer should be able to demonstrate safe work procedures which:

  • Are relevant to the business operations and scope of work

  • Demonstrate the process in which the business collects the relevant Safe Work Method Statements (SWMS) risk assessments, job safety analysis (JSA) Permits to work, prestart and other relevant documents, prior to commencing work

  • Demonstrate how documents are filed and retained for any site or future reference

  • Contain a description of the tasks, job steps and any associated hazards

  • Outline control measures and methods to eliminate or, reduce to as low as reasonably practicable any WH&S risks

  • PCBU can refer to any relevant Queensland Legislation, Codes of Practice or Australian Standards applicable to the scope of work being undertaken

Work Permits

  • Where relevant, the tenderer should be able to demonstrate work permits for the following types of work:

  • Excavation Permit

  • Hot Work (Hot Work Permit)

  • Lockout Permits (plant, electrical system, hydraulic etc)

Incident Reporting and Investigation

  • Tenderers should be able to provide evidence of the following:

  • Incident report and investigation form

  • Incident investigation procedure

  • Evidence of a standard Incident Report form

Plant Safety

  • The tenderer should have mechanisms and procedures in place for the identification of hazards, assessment of risks and the implementation of control measures associated with plant

  • Documented risk assessments for relevant plant or the PCBU risk assessment procedure

  • Copy of plant operator licences, permits

  • Register of plant requiring relevant registration

  • List of persons responsible for undertaking plant risk assessments

  • Plant maintenance, service and inspections forms

  • Pre-start daily safety inspection forms for plant

  • Plant prestart inspection check sheets for the plant audible and visual safety devices or systems (Reversing alarm, start-up alarm, amber lights, etc)

  • Plant fault reporting system and forms

  • Storage and the parking up process of plant while stood down, or while not in use

  • Plant security and the process used to ensure the plant remains safe while unattended on site

Hazardous Substances

  • Tenderer should provide evidence demonstrating safe handling and storage of hazardous substances:

  • Manifest or register of chemicals used by the company

  • Safety Data Sheets (SDS) for any chemicals to be used

  • Safe handling procedures, including use of personal protective equipment

  • Relevant Haz Sub training documentation

  • PCBU process for oil or other spill management including spill kits and relevant PPE

Manual Handling

  • The tenderer should be able to demonstrate evidence of:

  • Documented risk assessments for manual handling hazards

  • Systems used to control manual handling risks (eg. Lifting aids, work procedures)

Risk Assessment

  • Copies of any risk assessments relevant to the work (relevant generic risk assessments accepted for initial review)

Personal Protective Equipment (PPE)

  • List of PPE provided (policy and/or procedure)

  • PPE correct use training

  • Outline what PPE is required for a particular task and how it is monitored for correct use

Workplace Health and Safety Training

Training

  • Tenderer should be able to demonstrate evidence of:

  • Records of training and competencies of workers (licences, permits, certificates)

  • Records of ‘on the job’ training

  • Toolbox meetings conducted

  • Induction training program including any site specific

  • Record of all training and induction programs

  • Induction and training records An example is the required Construction Industry Induction card
