Company & Room Information

  • Are there first-aid materials in close proximity? Take a picture.

  • Are all emergency exits clearly labeled?

  • Is the classroom in trainee-ready condition? Take a picture.

  • Is the room isolated from noisy areas and separated by a closed door and/or "quiet" barrier? Take a picture.

  • Are the restroom and kitchen/break area in close proximity?

  • Does the location have adequate training facilities to conduct training and written tests? NCCER policy dictates that all rooms have at least 40 square feet of personal space per trainee and for testing, that each six-foot table have two or less individuals and each eight-foot table have three or less individuals.

  • What training materials are being used in class? This might include books, presentations, material examples, handouts, etc. Include examples.

  • Were there instructional aids utilized in class? If so, what types? Take pictures.

  • Are all test materials securely stored? Include a picture. NCCER policy dictates that all test materials must be in a secured location (locked filing cabinet, secure online storage, etc.).

  • How long are testing materials stored at the company location? NCCER policy dictates that all testing documentation must be stored for five years. MN Department of Labor & Industry policy dictates that all apprenticeship documentation must be stored for three years.

  • What is the company's retest policy? Include a copy. NCCER policy dictates that all re-testing must occur at least 48 hours after the last test attempt.

  • When are materials handed out to trainees? Before, during or after class?

Instructor(s) Information

  • What are the names, NCCER IDs and instructor expiration dates of all NCCER-certified instructors?

  • What is the name of the instructor being observed?

  • Did/Does the instructor have a professional appearance when he/she is presenting?

  • Did the class begin on time and were breaks laid out for the timeframe?

  • Did the instructor collect any prohibited items from the trainees? If so, how were they stored?

  • Was attendance taken and properly monitored?

  • Were the objectives of the lesson(s) stated and did the lesson(s) follow a logical path?

  • Were company policies and expectations discussed at the beginning of class?

  • Did the instructor display knowledge of the subject matter and confidence in his/her knowledge?

  • Were safety precautions taken during lab/shop exercises?

  • Did the trainees participate in class (ask/answer questions, share personal expereiences, etc.)? Ensure the instructor is allowing time for interaction and welcoming it when it does occur.

  • Did the course end on time?

  • Was there a course evaluation given at the end of class? Was it a paper copy or online? Include a copy.

  • How was the instructor "control" over the classroom? Were students allowed to speak at will or were there restrictions? Did the instructor have control of the classroom?

  • What is the instructor's (or company's) policy for grading tests and reviewing the materials with the trainee?

  • How did the instructor deliver the training? Did he/she seem nervous? Were there areas that he/she excelled? Did he/she focus on the trainees?

Written & Performance Testing

  • Did the instructor follow company guidelines when conducting the written and/or performance test(s)? Include company procedure if available. NCCER policy dictates that all written testing must be closed book/note and no outside assistance be given. NCCER policy dictates that all performance testing must be conducted one-on-one and in a safe manner.

  • Are tools, equipment and other materials regularly inspected?

  • Does the location have proper facilities for performance testing? Include pictures and descriptions.

  • What is the procedure for extra tests and/or used scrap paper, notes, etc. that were created during the testing?

  • Was a written and/or performance test observed? If so, include information related to the observation. If a test was not observed, have an instructor walk through the process and observe the steps/procedures.
