Are sharps containers located near the areas where sharps are used?

Are in use sharps containers assembled correctly with finger guard?

Are sharps containers only filled to the designated line?

Are Qlicksmart devices available for scalpel blade removal?

Are loose blades/used needles sitting around lab areas?

Obtain a list of s4 drugs ordered by the floor in the last 12 months from purchasing.

Does each group that uses s4 drugs have a drug log?

Provide s4 stickers to groups that require them (note: these are not mandatory)

Are users of s4 drugs aware of the requirement to log usage?

Are the drug logs kept up to date (compare groups list with purchasing list)?

Are there any obvious lighting issues?

Is the laboratory floor free of cardboard boxes?

Is the laboratory free of wooden pallets?

Are there excess eskys?

Is the lab free of unnecessarily stored items (eg old broken equipment, packing materials)?

Are the corridors unobstructed?

Are all self-closing doors operating effectively (ie closing without assistance)?

Is the wooden lab doors varnish still in good condition/providing an effective seal?

Is the lab free of material/absorbent chairs?

Is benchcoat in good condition and not taped to benches?

Are communal work areas neat and tidy (ie gel stations, staining areas)?

Check all incubator and other gas line fittings.

Are there any visible or audible signs of leaks (ie cracked tubing, hissing sounds)?

Is the gas line tubing in good condition?

Are all CO2 incubator regulators set to the appropriate pressure (3-5psi)?

Is all tubing leading into incubators and other devices appropriately fitted with clamps?

Request an up to date list of researchers on your floor with GMO dealings from TRI WHS.

Do all lab fridges and freezers have a purple GMO storage sticker?

Do all groups that work with/store GMO's have the location of their storage records listed in the grey OGTR folder?

Do all lab groups that work with/store GMO’s have up to date storage records?

Can lab groups differentiate between their GMO and non-GMO material (normally done via labeling)?

Select a random group to do a 'take me to your GMO' walkthrough.

Is the temperature within the acceptable range?<br>- A temperature logger is available from WHS if required.

Is the area free from noise hazards?

Are the tea rooms and meeting rooms clean, neat and tidy?

Are the tea rooms and meeting room’s furniture in good condition and adequate (ie enough chairs)?

Are electrical fittings, cords and power boards in good condition and free of visable signs of wear?

Are people seated correctly at workstations?

Do all in use workstations have ergonomic chairs with a 5-castor base?

Is the area free of trip/slip hazards?

Is the floor in good condition and clean?

Is the general work area clean with the bins emptied regularly?

Are items stored so they are not at risk of falling?

Is the lighting adequate?

Does the general ventilation appear sufficient?

Are the cold rooms free of mould?

Are the cold rooms free of cardboard?

Are all items in the cold room labelled with researchers name/group?

Are biological items (ie agar plates, samples) double contained and labelled?

Are electrical items and fittings free from signs of wear/corrosion?

Is freezer signage and emergency contact details displayed on freezer doors and up to date?

Do all freezers have emergency contact details listed?

Do all lab fridges and freezers have a biohazard stickers and purple GMO sticker?

Do all alarmed freezers have the BMS point number listed on the front?

Are freezers kept free of excessive ice build-up (especially around doors and seals)?

Are all in-use laboratory suction lines fitted with an uncontaminated 0.2µm filter?

Are filters fitted in the correct orientation?

Is vacuum line tubing in good condition (ie no kinks or collapsed sections)?

Are vacuum lines free from mould or bacterial growth?

Do vacuum trap bottles contain disinfectant solution (ie biodyne)?

Are vacuum trap bottles free from mold or bacterial growth?

Check testing records (should be located in spill response cupboard). Are eyewashes and safety showers are being tested regularly?

Is annual testing conducted in accordance with AS4775?

Check under the eyewash covers. Are they free from signs of wear/damage (eg cracking, broken plastic)?

Is the area under the safety shower/eyewash stations free of stored items or other obstructions?

Is disinfectant available and in date at hand wash sinks?

Are hand wash sinks free of laboratory items?

Are decanted chemical bottles clearly labelled with name and risk phrase?

Do all hazardous chemicals have a TRI orange barcode?

Record details and arrange barcodes for those which don't

Are the storage cabinets in good condition?

Are the fume cupboards free of stored hazardous chemicals?

Are there only flammables (class 3) liquids stored in the flammables cabinets?

Are there only corrosives (class 8) liquids stored in the corrosives cabinets?

Are hazardous chemicals segregated by their DG class in storage cupboards and areas?

Are hazardous and non-hazardous chemicals segregated in storage cupboards?

Do hazardous liquid chemicals have spill containment (eg stored on trays)?

Are there large volumes (20L) of class 3 outside a flammables cabinet?

Are there collections of >1L Haz chems outside of designated storage cupboards or cabinets?

Are all fire extinguishers, hose reels and hydrants within test date (last 12 months)?

Are the fire exit signs working and visible?

Are fire exits free from obstruction?

Are all other exits and corridors free from obstruction?
