  • Use this audit to assess the following aspects of this site as Safe, At Risk or Not Applicable. Unsafe response indicate that attention is required to bring items to a satisfactory standard. Notes and photos should be attached where appropriate, particularly to At Risk items.

Work Areas

  • Name of the Competent person on-site at all times?

Personal Protective Equipment (PPE)

  • IS PPE available and being used on site?

  • Hard Hats (available, worn by crew and in safe condition)

  • Safety Glasses (available, worn by crew and in safe condition)

  • Safety vest or hi viz outer wear used by crew

  • Face Shield / Goggles (worn when cutting with power saw)

  • Respiratory Protection (Voluntary Use Appendix D to document)

  • Appropriate Clothing (Hot Tar Applied-Leather gloves and face shield at kettle)

  • Gloves (available, worn by crew and in safe condition)

  • Foot Protection (fit for task, worn by crew and in safe condition)

Fall Protection Equipment

  • Is PFAS being used to protect from Falls?

  • Fall protection equipment inspected prior to use and found with no defects?

  • Appropriate for task and used properly by crew where applicable

  • Suitable Anchorage Points (provide at least 5,000 pound anchorage)

  • If guardrail or parapet walls is the height at least 39 inches

Ladders

  • Are ladders used on site this inspection?

  • Ladders are well maintained and inspected prior to use?

  • Extension Ladder base, secure and level and top secured from movement

  • Proper heavy duty or extra heavy duty type 1 or 1A ladders used?

Tools -Power and hand held

  • Are torches being used at this inspection site?

  • Are all LP tanks secured from falling?

  • Have regulator connections been tested for leaks?

  • Is at least a 10 pound ABC fire extinguisher readily available?

  • Has a Hot work permit been completed prior to starting scope of work?

Electrical Safety

  • Will electrical cords and tools be used?

  • Ground fault circuit interrupters in use

  • Extension Cords are not causing trip hazards or used inappropriately

  • Extension cords have been inspected for damage before use?

Equipment Inspections

  • Will equipment be used during this inspection?

  • Have equipment inspections been completed (forklift-aerial lift-scissor lift)

  • Are operators licensed where required and trained to operate safely?

  • Is barricade installed at ground level to protect from falling objects from overhead work?

People Safety

  • Do you wish to check work crew safety knowledge at this inspection?

  • Approach at least one team member and record their responses to the following. Make clear this is not an assessment of the team member themselves, but rather the safety procedures on site.

General Comments

  • Has toolbox talk been completed this week (take photo)

  • Add any additional relevant photos

  • Have all required permits been completed prior to task
