Essentials

1. OHS Displays and posters

1.1 Is the 'Injury Advisory Support Line' flyer displayed?

1.2 Is the 'If you are injured' poster displayed?

1.3 Is the OHS notice board maintained and documents current?

1.4 Are the emergency procedures displayed? (Located at entries and exit)

1.5 Are the list of HSRs displayed? (OHS notice board)

1.6 Are the Designated First Aiders displayed?

2. Housekeeping

2.1 Are aisles / walkways clear of obstacles?

2.2 Are floors free from slip trip hazards?

2.3 Are workstations tidy and free from clutter?

2.4 Are work areas free from rubbish and clutter?

2.5 Is there adequate lighting?

2.6 Is the adequate heating, cooling and ventilation?

2.7 Is there sufficient storage space?

3. Amenities

3.1 Are toilets clean and operational?

3.2 Are kitchenettes clean and tidy?

3.3 Is there is a clean supply of water for drinking available at all times?

3.4 Number of bins suitable for segregation / recycling?

3.5 Are rubbish bins available and covered?

3.6 Are change rooms / lockers clean and tidy?

3.7 Is air conditioning, where installed, working correctly? Correct temperature maintained?

4. Electrical

4.1 Are plugs, sockets and switches in good repair?

4.2 Are leads and cords in good repair?

4.3 Are testing dates current?

4.4 Are double adapters in use?

4.5 Lights and fittings - working, undamaged and clean?

5. Emergency

5.7 Are emergency personnel identified and trained?

5.8 Are first aid kits available and signposted?

5.8a Is the Defibrillator showing a green arrow and all pads and equipment available in original packaging?

5.9 Fire extinguishers available and signposted?

5.10 Fire extinguishers tagged and tested?

5.11 Is signage in good condition?

5.12 If applicable, are duress alarms operational? (conduct test)

5.1 Are emergency exits signed and illuminated?

Emergency exits has at least 1m wide and 2m high clear pathway to exit

5.2 Are emergency exits free from obstruction?

Are emergency exits ( including fire-isolated passageways, ramps and stairs) free from obstruction?

5.3 Are the paths of travel from emergency exits to the road / safe open space area obstructed?

5.4 Are emergency exits fitted with electrical devices free from obstruction (sliding / hinged auto doors)?

5.5 Are exits door simple auto operate from the inside and can be opened readily with a key?

5.6 Are the alarm and Communication systems adequate?

Manual handling, Chemicals, PPE

6. Manual Handling

6.1 Have all manual handling risk been assessed?

6.2 Are manual handling control measures implemented?

6.3 Have employees received manual handling training?

6.4 Are mechanical aids used for manual handling controls?

7. Chemicals & Hazardous Substances

7.1 are (material) safety data sheets available for all chemicals?

7.2 Are safety data sheets current?

7.3 Are chemicals stored according to applicable Safety Data Sheet?

7.4 Are all chemicals labelled correctly?

7.6 Has a Hazardous substances register been developed?

7.5 Are chemical storage areas signed appropriately?

7.8 Is protective clothing available / used with chemicals?

7.7 Has chemical use been risk assessed?

7.9 Are spill kits available and clearly signed?

7.10 Are the appropriate first aid . emergency control measures in place? e.g eye bath, extinguishers

7.12 Are the chemicals required for tasks onsite?

7.11 Where chemicals are decanted, is bunding in place?

8. Personal Protective Equipment

8.1 Is appropriatePPE available and used?

8.2 Are employees trained in the correct use of PPE?

8.3 Is PPE well maintained and in good repair?

8.4 Are there appropriate signage indicating PPE requirements?

8.5 Is there appropriate storage?

Mechanical, Ergonomics and Tools

9. Mechanical Plant & Equipment

9.1 Is all plant and equipment in good repair?

9.2 Is plant and equipment serviced as per manufacturer's requirements?

9.3 Are records of all servicing available?

9.4 Are employees trained in the use of plant and equipment?

9.5 Do employees hold the appropriate and current licences to operate plant and is there a copy on file?

9.6 Has all plants and equipment been risk assessed?

9.7 Are records of pre-start checks maintained and available?

10. Ergonomics

10.1 Are workstations set-up / designed to minimise bending, twisting and lifting?

10.2 Seats / chairs in good condition?

11. Hand Tools and Machine Guards

11.1 All tools safely stored when not in use?

11.2 Are defective tools in evidence or in use?

11.3 Guards are in place, as intended?

11.4 Guards are in good condition and working?

Lifting Equipment, Vehicles & Pressure Vessels

12 .Lifting Equipment

12.1 Equipment identifies and Working Load Limit clearly marked?

12.2 Chains / slings identified and inspection up to date?

12.3 Register of lifting equipment maintained?

13. Vehicles

13.1 Daily checks completed?

13.2 All identified defects rectified?

13.3 Vehicles being operated in a responsible manner?

13.4 No excessive fume emissions?

14. Pressure Vessels and Compressed Gas Cylinders

14.1 Pressure vessels and piping in good condition, no obvious corrosion or oil leaks?

14.2 Are there any evidence of perished hoses on cutting torches?

14.3 Flashback arrestors fitted?

14.4 Gas secured and stored clear if ignition sources?

External Area / Carpark and sign off

15. External areas / Carparks (if applicable to your work area)

15.1 Are there any slipping / tripping hazards?

15.2 Is there adequate lighting (dusk / dawn)?

15.3 Are walkways / paths clear of obstacles and debris?
