1.0 Are Contractors operators properly certificated or qualified?

Verify that the Contractor has checked to see that workers operating earthmoving plant such as front-end loaders, front-end loader/backhoes, skid steer loaders, excavators, dozers and draglines have an appropriate qualification card or certificate and the Contractor has determined employees have the necessary competency.

Contractors must have had operators show them their certificates and qualifications, and have recorded the details on the site register. If no formal certificates are required, ensure the Contractor has a process to determine employees have the necessary competency to safely operate the plant.
2.0 Has Contractor given operators training in the use of the particular machine/s they are operating?

Certificates of competency mean the operator has been assessed as generally competent to operate the classes of plant covered by the certificate. But it does not necessarily mean the operator is familiar with the particular make and model or the particular attachments being used with the plant. Verify the Contractors system to ensure training in the particular plant and models they are using on site.

Verify how the Contractor makes sure operators are familiar with their machine and attachments, that they have the operator's manual and that they understand the safe operating procedures for the machine and the particular work.
3.0 Location of all underground services

3.1 Has the contractor contacted and received the dial before you dig information for the site and surrounding area

3.2 Before the start of any excavation, has the contractor ensured the exact location of any underground electrical cables, Gas lines, water, Sewerage and telecommunications cables

3.3 Review the information the contractor has to determine whether the contractor has investigated the presence of any underground services and is not relying solely on site plans and drawings

3.4 If the excavation is over 1.5 m verify the contractor that has lodged Notice of Intention to commence the trench with Worksafe
4.0 Safe use of Earthmoving plant

4.1 Verify the contractor has a current skills register on site so that the competency \ training endorsements of the plant operators can be confirmed

4.2 Confirm the Contractors system to make sure all earthmoving plant is properly maintained and fully serviceable

4.3 Check that excavations are not undermining existing buildings or temporary structures such as Scaffolds or Formwork

4.4 Ensure spoil is being kept at least a half a metre back from the edge of the trenches and the earthmoving plant is a safe distance from the edge of the excavations

4.5 Ensure Contractors supervision requires that plant parked overnight alongside roads or in a public space has systems to make sure plant is securely locked with buckets fully lowered and appropriate traffic warning lamps and barricades in place
5.0 Safe management of trenches and excavations

5.1 Verify the Contractor has lodged a Notice of Intention to commence the trench with Worksafe.

5.2. Are trenches or excavations over 1.5 m deep being managed correctly with battering, shoring or trench shielding

5.3 Ensure that if in the event of an incident or accident safe egress is available from the trench or excavation

5.4. Ensure that any shoring has appropriate materials, min F8 hardwood or steel props

5.5. Ensure the Contractor is wearing appropriate PPE and in accordance with his site specific safety plan & JSEA's
6.0 Confined or Restricted space requirements

A Contractors assessment has been carried out and ALL FOUR assessment criteria to determine a Confined Space (A) Enclosed or Partially Enclosed (B) Normal Atmospheric Pressure (C) Restricted Entry or Exit (D) Contaminants / Unsafe Oxygen or Engulfment have "Yes" as an answer and a confined space procedure is planned or underway

7.0 Fall Safeguards

7.1. Ensure Contractor has appropriate covering, barricading or isolation to prevent people from falling into the excavation

7.2. Ensure the Contractor never leaves an excavation unprotected when unattended
8.0. Access and Egress

8.1. Ensure the Contractor has appropriate industrial grade portable ladders combined with fall limiting devices if required

8.2. Does the Contractor have a backup plan for removing injured personal from the excavation other than normal egress.
9.0. Working alone

9.1. Confirm the Contractor has a policy in place to ensure workers are never in an excavation without someone close enough to render assistance if needed
10.0 Inspection of open excavations

10.1. Confirm the Contractor has suitably trained supervision staff overseeing the excavation/s

10.2. Confirm that the trained staff inspect the excavation/s on a regular basis and record the inspections

10.3. Confirm the Inspection documentation has checks for the condition of shoring, battering and trench walls for signs of fretting, slipping, slumping or ground swelling

10.4. Confirm the Contractor has in place a system that where necessary, repair works are carried out before allowing works to continue

Contractor is to review the comments, and consider the recommended actions within this Audit. The Contractor needs to demonstrate they are complying with their obligations as an employer, what specific remedial action they propose for this site, and if any overall modifications are required to their Health and Safety Plan / safety procedures / supervision. Prompt corrective action is required to eliminate any agreed deficiencies, and areas of any disagreement need to be further explored in writing. All “failed responses” need to be addressed/resolved.
