  • Is the Operator signed off on the Alert?

  • Please enter employee name and employee number.

  • Is section C) completed correctly by initiator?

  • Please contact initiator to correct.

  • Has the Department Manager or Designate signed off on D)?

  • Can the operator explain the steps for their station related to alerts?

  • Is Operator performing task according to the alert?

  • Contact Teamleader to advise of failure to follow alert.

  • Is the Quality Alert reflected on the Quality Triangle and/or Team Performance Board?

  • Please notify the Teamleader immediately.

  • Observe Repair Cell - is repair being inspected before pack? Take picture of repair and label.

  • Was repair transacted properly through DDC?
