Demo One Thorough Visual Inspection Form

Objective

  • To provide confirmation the Demo One Site Supervisor has carried out a thorough visual inspection of the Enclosure (or Work Area) and the area is clear of visible debris and dust. This form to be passed/emailed to Analyst before the 4-stage clearance starts.

Enclosure and Visual Inspection Details

  • Name and/or number of enclosure being inspected

  • Size of enclosure (see Plan of Work) in metres cubed (L x W X H)

  • Has full access of the site been obtained (where necessary)?

  • Has a new pre-filter been installed in the NPU(s)?

  • Have all ACM removal locations been checked and certified as free from asbestos?

  • Have all surfaces floors, walls, ceilings and other items been inspected and confirmed as visually clean?

  • Have all ledges, sills, higher level surfaces including voids (where appropriate) been inspected and confirmed as visually clean?

  • Have all cables, wiring, pipework, ducting etc. been inspected and confirmed as visually clean?

  • Have all items and equipment brought into and/or remaining in the enclosure, for the clearance procedure (including scaffolding and large waste items) been checked and confirmed as visually clean?

  • Photograph of inside the enclosure passed for 4-stage clearance

Confirmations

  • Start time of Visual Inspection

  • Finish Time of Visual Inspection

  • I certify I have carried out a thorough visual inspection of the enclosure/work area and confirm the area is visually clean and ready to be made available to the Analyst to commence the independent 4-stage clearance for the certificate of reoccupation.

  • Name and signature of Site Supervisor

  • Form to be handed/emailed to Analyst.

  • Name and signature of Analyst
