Individual Crane Operator was wearing PPE properly:

  • Manager performing evaluation

  • Employee being evaluated

  • 01. Hard hat. (May be removed while in cab)

  • 02. Reflective devices. (Lime green, breakaway vest)

  • 03. Steel toed safety boots.

  • 04. Safety glasses with side shields. (Clear color must be worn at night)

  • 05. Attends daily job briefing.

  • 06. Visual inspection, records inspection.

  • 07. Tests operation of pulleys, locks and arms.

  • 08. Moves inside crane path if outside normal crane path, spotter must be available and in radio contact with operator.

Loading Trailers

  • 09. Positions arms to proper lifting positions without striking arms on side of unit.

  • 10. Engage arms for lift.

  • 11. Raise nose for sand shoe to clear contact with ground allowing grounds person to properly adjust height.

  • Automatic Failure if rear wheels off of ground, lift motion not stopped while adjustment is made to legs.

  • 12. Lift unit high enough to clear car.

  • 13. Position unit as directed by grounds person.

  • 14. Once kingpin is locked, perform “secondary lift” or “pull”.

  • 15. Position tandems.

  • 16. Raise arms. Remain over unit until arms are completely folded in stored position, if applicable.

  • Automatic Failure if visual contact between lift operator and grounds person is lost.

  • 17. Loads trailers at company standard of 2 minutes per unit or faster.

Unloading Trailers

  • 18. Position over trailer, lower arms and position properly for lift.

  • 19. Move kingpin out of jaws, slowly not to damage auxiliary lock.

  • 20. Lift high enough to clear car and place rear tandems on ground, leaving sand shoes off ground allowing grounds person to adjust height.

  • 21. Lower unit to ground. Release arms and fold in full folded position.

  • (Automatic Failure if chassis with containers are not loaded according to manufacture markings)

  • 22. Unloads trailers at company standard of 2 minutes per unit or faster.

Loading Containers

  • 23. Position over unit, engage twist locks.

  • 24. Lift unit slowly, allowing hostlers, if connected to chassis, to depart.

  • 25. Place unit on pedestals. Adjust as necessary.

  • 26. If top unit on double stack, ensure all IBC’s are in place and grounds person is clear.

  • 27. Loads containers at company standard of 2 minutes per unit or faster.

Unloading Containers

  • 28. Position over container engage twist locks.

  • 29. Lift unit to clear pedestals or IBC’s.

  • 30. Place unit on ground or chassis.

  • Automatic Failure if unit is lifted and left in the air while awaiting chassis.

  • 31. Adjust unit on chassis as necessary.

  • 32. Unloads containers at company standard of 2 minutes per unit or faster.
