Written program specifies:
• Roles & responsibilities;
• PPE requirements including inspection frequency and method;
• Tool requirements including inspection frequencies and method;
• Energized work permit process is clearly defined (E8.4-3.6);
• Arc flash hazard evaluation and protection;
• Employee training requirements.

A revision log shall be included as part of the written program.
Review written programs.
Requirements for PPE and tools must be specific for agency approvals, voltage rating, category (multi-meters and arc flash PPE) and other characteristics (e.g. two color insulation on hand tools to aid with inspection). PPE requirements for tasks performed must be specific. An inventory of tools is not necessary as long as the characteristics are specified.
-
PPE and tools needed for safe electrical work practices are available.
-
Written program is reviewed annually and updated as necessary. The revision log has been maintained to reflect the annual review and any other changes made to the written program throughout the year.

Supervisors "Qualified" and "Task Qualified" employees receive formalized training, including exams, upon assignment and every three years thereafter. Effective refresher training is provided during other years
-
Trainers are competent and qualified to train on the subject matter.
-
Site maintains a list of qualified employees (task qualified or fully qualified) which includes the date of last formalized training.
-
PPE and tools are inspected per written program..
-
A formal arc flash analysis (engineering study) has been completed and has been updated every five years or when major changes take place. Electrical panels are properly labeled and qualified employees trained on the hazards and how to protect themselves. Appropriate PPE and tools are available and in good condition.
-
An effective predictive and preventative maintenance system is established and implemented for the site's electrical distribution systems.
-
Only trained and qualified employees perform electrical work. "Qualified" and "Task Qualified" employees perform only the work they have been trained and authorized to perform.
-
Safe electrical work practices are strictly followed. At a minimum, sites must comply with NFPA 70E and 29CFR 1910 Sub-Part S.
-
Energized electrical work permit process is strictly followed.
-
Work on energized equipment does not occur.
Exception: Diagnostics (taking meter readings and performing trouble shooting).
