THE CERTIFICATE OF COMPLETION MUST BE ATTACHED TO THE CONCRETE SAFETY MANAGERS LOG UPON RECEIPT FROM EMPLOYEE. EMPLOYEE CAN NOT COMPLETE RETRAINING DURING WORK HOURS. EDT'S MUST BE COMPLETED ON THE DAY IT WAS ISSUED.
