Notification Of Service Mains Replacement - NOSMR form

1. Clear and correct job details. Date, work order, meter number and address completed and confirmed.

2. Tests and results completed and recorded clearly as required. NIT test, polarity, IR test, phase rotation, earth integrity, continuity, final test.

3. Clear and accurate diagram including POA distances from boundaries, property alignment, network pole connection location & details, street alignments, road clearance pole location & details, meter box location, service lengths and service clearances at centre of carriageway and kerb completed.

4. NOSMR signed by AUP and witness. AUP number provided.
NETWORK - POCC

1. Correct connectors used on the network mains and attached as required.

2. J-hook used and clear off any live parts of network (i.e. streetlight, UGOH)

3. Network pole number matches the NOSMR form details.
-
4. Open points and side of service connection matches NOSMR form details.
-
5. Old connectors removed from network mains.
-
6. Strain clamp installed correctly and service tensioned with no excessive sag visible.
-
7. All redundant materials and rubbish removed from site.
ROAD CLEARANCE POLE

1. Road Clearance Pole number matches the NOSMR form details.

2. Correct connector used and attached as required.

3. Strain clamps installed correctly and service tensioned with no excessive sag visible.

4. All redundant materials and rubbish removed from site.
POINT OF ATTACHMENT
-
1. Q-hook used and installed correctly.
-
2. Strain clamp installed correctly and service tensioned with no excessive sag visible.

3. Sleeving used to cover consumer mains at the POA.
-
4. Neutral tag attached to consumer neutral conductor at POA.
-
5. POA Bond installed where required and metallic connection sprayed over.
-
6. Old insulators and hardware from customer's POA removed.
-
7. All redundant materials and rubbish removed from site.
METER BOARD
-
1. Using proximity tester check and verify no voltage exist on the meter box enclosure and it is safe to proceed.
-
2. Check and verify meter number matches the NOSMR form details.
-
3. Check if all fuses, covers, enclosures are sealed.
-
4. All redundant materials and rubbish are removed from site. Check bottom of the meter box.
FINAL CHECK AND SIGN OFF
-
1. All changes from Work Order noted and variation recorded.
