Evacuation/Emergency and Incidents

  • Emergency Identification - what sounds to listen for, what calls over radio etc

Personal Protective Equipment

  • Personal Protective Equipment (PPE) must be worn by all persons on site. Ensure the following minimum PPE requirements are being met:

  • Hi Visibility Hard Hat/Helmet (yellow or orange)

  • Hi Visibility Vest/Shirt/Jacket (yellow or orange)

  • Steel Capped Boots - with ankle support

  • Is there any other PPE required due to the purpose of the visit? If so please check below or list in the box provided.

  • Hearing Protection - Grade 5

  • Safety Gloves

Operational Areas

  • The following areas are to be explained and any questions (including the response) is to be recorded

  • Review if the Visitor/Contractor requires to be escorted partly or the for the whole visit

  • Communication Requirements - radio (correct channels), visual signals etc with ground crew, machine operators

  • Chain Shot - DANGER ZONE!

  • Hazards - identify if any risks or hazards may effect their visit

  • Parking/Vehicles - where to park, facing exit etc

  • Smoke free Workplace Policy - cigarette butt containers, no smoking areas (vehicles, staff container, machines)

  • Housekeeping Requirements - rubbish in bags provided or take away, spill response requirements

  • Is the Visitor/Contractor required to complete any other paperwork? (eg:- site work permit, site specific hazard ID, Daily Toolbox, LOTO form, etc)
