1.0 ENGINEERING & ADMINISTRATIVE CONTROLS

  • 1.1 For load bearing falsework and formwork confirm the contractor has used a competent designer and / or manufacturer / supplier

  • 1.2 Confirm there is a current copy of the design drawings and loading calculations available on site

  • 1.3 Confirm that the Contractor is aware of the curing times and ensure these times are strictly adhered to prior to the formwork or falsework being stripped or dismantled

2.0 FALSEWORK CONSTRUCTION

  • 2.1 Check that the Contractors Falsework is in serviceable condition

  • 2.2 Verify the Falsework system is the same that has been detailed and specified for the job

  • 2.3 Confirm the Contractor has checked the system is on firm foundations and has been constructed in accordance with design plan

  • 2.4 Ensure any adjustable building props are secured so they cannot collapse when released

3.0 CONTRACTORS METHOD OF LAYING FORMWORK DECKING

  • 3.1 Does the method being used safeguard the workers from falls

  • 3.2 Confirm safe access and egress to work area has been established

  • 3.3 Has perimeter edge protection been provided<br>

4.0 LIFTING AND SECURING VERTICAL SHUTTERS

  • 4.1 Ensure proper lifting procedures are followed when crane lifting shutters, secure slings, taglines etc

  • 4.2 Confirm the Contractors methodology to securing shutters is safe, properly constructed scaffolds are required for work platforms

5.0 CONTRACTORS STEEL FIXING

  • 5.1 Does the contractor ensure plastic protective caps are alway placed on ends of starter bars to safeguard workers

  • 5.2 Verify the contractor is providing properly constructed scaffolding for steel fixers

  • 5.3 Confirm that steel fixers have and are wearing PPE that has been nominated in the JSEA or SWMS

6.0 CONCRETE PUMPS

  • 6.1 Confirm the contractor has checked that any boom type pumps comply with worksafe requirements prior to commencement of work

  • 6.2 Confirm the contractor has checked the setup location

  • 6.3 Confirm the pump washout area is appropriate.

7.0 CONCRETE KIBBLES

  • If Kipples are NOT being used please use checkbox

  • 7.1 Contractor should check all competency tickets to ensure a qualified Dogman or Rigger is directing the use of the Kibble

  • 7.2 Discuss the need to slow release concrete to not point load the formwork or create boom whip

  • 7.3 Contractor must ensure proper handling procedures are in place within the JSEA prior to work staring

8.0 CONCRETE VIBRATORS

  • 8.1 Verify prior to the pour the contractor has the appropriate vibrator and being used by a competent person

  • 8.2 Electrically powered types should be protected with RCD's

  • 8.3 Verify that petrol driven vibrators are properly ventilated, dot not use inside poorly ventilated areas

9.0 CONTRACTORS CONCRETE WORKERS

  • 9.1 Confirm the contractor has checked there are no open sides or penetrations where a worker could fall more than 2 metres

  • 9.2 Confirm the contractor has assessed any requirements for temporary guard railing or perimeter scaffolding

FORMWORK STRIPPING

  • Verify the Contractors supervision has areas barricaded if stripping underside work

  • Confirm contractor has adequate properly constructed scaffold for work to operate from when stripping

General Comments

  • General Comments : Contractor is to review the audit, and consider the recommended actions within this Audit. The contractor needs to demonstrate compliance with his obligation as a employer, what specific remedial action he proposes for this site and if any overall modifications are required to his Health and Safety plan / Safety Procedures / Supervision. Prompt corrective action is required to eliminate any agreed deficiencies. Any areas of disagreement need to be further explored in writing. All follow up items need to be addressed / resolved.
