  • Machine Induction Checklist
    - Utilise to induct a Machine into your Worksite if it is New, Leased or Returned after a minimum of 1 week out of commission (moderate to major repairs or modification)

  • This form is to be completed by the Operator and Inspector when a piece of machinery is purchased or leased. All corrective actions must be noted on this form, corrected, then signed off as corrected prior to the machine being used. The fully completed form, including all corrective actions must be reviewed and signed off by the Principal (Owner/Operator).

    Note: A corrective action is any modification or repair required to ensure machinery meets ACOP standards (section 6: Mobile Plant).
