EVALUATION

  • Are all activities performed in compliance with an accepted JSEA that is still adequate for the current tasks & conditions?

  • Do all persons appear to understand the requirements of the JSEA & have signed off?

  • Are exclusion zones implemented using barricades of at least 900mm height to prevent unauthorised interaction with the trench?

  • Is each bench cut into the side of the trench no higher than it is wide unless a geo-technical engineer has approved a greater height in writing?

  • Is battering less than 45ยบ with a maximum vertical face of 1.5m unless otherwise approved by a geo-technical engineer in writing?

  • Is there documented evidence that design specifications for engineering controls such as shoring support structures have been prepared by an appropriately qualified person?

  • To reduce the risk of trench collapse, has spoil, mechanical plant, vehicles & materials been placed so that they do not encroach on the zone of influence? Alternatively, is there documented evidence that the support system has been designed to accommodate such additional loads?

  • Whether the trench is defined as a confined space or not, are control measures in place to eliminate or control risks associated with contaminated, explosive, flammable or oxygen deficient atmospheres that have been applied to the trench?

  • Is an excavation permit in place?

  • Is there documented evidence that the following conditions that may result in or be an indicator of trench collapse are being monitored?

  • Shoring becoming unstable

  • Angle of batter remains sufficient to prevent collapse

  • Cracks appearing near the edge of the trench

  • Surface soil falling into excavation

  • Water seeping into excavations from its side walls or base

  • Heaving or swelling of the ground at the bottom of the trench

  • Subsidence alongside the excavations

  • Has a complete record search of existing services been undertaken & validation of records has taken place prior to work starting?

  • Are there any other risks or issues associated with the activities observed?
