Construction Phase Plan

  • Does the site have clear & relevant information to safeguard the workers on site?<br>Has the information been approved & formally issued to the site personnel?<br>Has it been communicated, eg at site induction <br>Has it been recorded, eg site register<br>Are recent site inspections in the site pack?

General duties

  • Is there a method of reporting unsafe conditions on site, eg near miss forms
    How is the near miss system accesses by site personnel & how frequently has it been used on site?

Competence

  • Can individuals on site prove their identity & competence?<br>Do the individuals match the site induction register?<br>Who has responsibility on site for all workers? Is this clear?<br>Has the principal contractor site supervisor recently signed into site?<br>

Access/egress

  • Is there suitable access/egress from excavations or other confined spaces,including in an emergency?<br>Does this match permits in place?

  • Has the Work area/site been suitably secured from unauthorised personnel, including members of the public?<br>Is the site perimeter clearly defined?

  • Is there enough room on site from start to finish for all expected plant, equipment & materials?<br>Consider diggers, dumpers, vans, delivery lorries, etc

Design

  • Are there design drawings relating to the work, including reference to standard designs in Cadent policy & procedure, eg Maint 5028 pin brazing, etc<br>Does the design and/or specific policy/procedure eliminate the risk to the workers on site?<br>Is there a sequence of works, eg method statement, project plan? Is the sequence being followed?

Site tidiness

  • Is the work area clean & today and free from trip hazards & sharp objects, eg timber, nails?<br>Has slipping and stranding of vehicles been considered, eg temporary road ways in fields, regular road sweeping, etc?

Temporary Works

  • Are the temporary works being dismantled in a planned & organised manner? <br>Is there a plan being followed, eg permit to strike

  • Are materials, plant & equipment being kept a unit able distance from the edge of the excavation?<br>Consider how vehicles will stop, how backfill will be stored, how equipment in use will be stored

  • How frequently are the trenches & other excavations inspected?<br>Is this recorded on the permit?<br>If there are concerns raised at inspection, has a report been produced & issued to the team Leader and/or supervisor?<br>Can the team Leader tell you who the TWcompetent person is?

Cofferdams & caissons

High Risk Utilities

  • Are there drawings of all utilities which have been printed or refreshed within the last 3 months?<br>Has there been a suitable mark up on site, including overhead hazards?<br>Is the work being carried out in a way which avoids the need to work in close proximity to the utility? Does the work match the design drawings?<br>Consider 3rd party pipelines, cables, MP/IP gas

Prevention of drowning

  • If there is a risk of drowning on this project are there suitable controls in place to prevent this happening? E,g edge protection, pumping, etc<br>Is there a site specific rescue plan?<br>Consider close proximity to deep water, deep excavations with high water table

Traffic routes

  • Is there enough space for all vehicles expected on this site to pass safely to their required location?<br>Are pedestrians & vehicles segregated? Consider all vehicles, eg vans, delivery vehicles, reinstatement, vac ex, digger, dumper, etc<br>Are there vehicle stops in place where required?<br>Are the welfare facilities away from traffic routes?<br>Are deliveries remote from the work area?<br>Are there adequate signs & systems in place so that drivers easily understand the rules for movement on site?<br>Is the site clear from obstruction to vehicles, eg banks, bunds, static plant, excavations?

  • Is the work area clear from vehicle movements?<br>Have delivery drivers been inducted to site, eg materials,plant, barriers, reinstatement?<br>Can the driver of any vehicle demonstrate competence, including towing?<br>Is the plan of the site showing vehicle arrangements included in the pack & clear to all on site? Eg vehicle movement plan

Prevention of risk from fire, flooding or asphyxiation

  • Is there a risk assessment that considers all hazards and their impact?<br>Consider who can be harmed - workers, visitors, adjacent land users, members of the public <br>How might hame occur - think of what could go wrong<br>Adequate control - is the control measure robust or could more be done ?<br>

  • If the work is covered by SCO procedure, is the competent person on site?<br>Has the correct permit Roy been established?

Emergency procedures

  • Is there a suitable emergency plan documented in the site pack?<br>Does the plan consider the work site and adjacent areas that may be affected? Eg places of high impact, difficulty with evacuation, clearly marked emergency exits<br>Are there named individuals in the site pack to lead in an emergency?<br>Is the team aware of what to do in an emergency?<br>Is the local A&E hospital shown in the site pack, including directions?

Essential safety equipment

  • Is all safety equipment relevant the task available, in good condition & sufficient for all workers on site?<br>Consider:<br>BA, escape sets<br>Fire extinguishers, no smoking signs<br>Atmospheric & personal monitoring <br>Continuity bonds<br>Pumps<br>Volt stick<br>Lighting<br>First aid, including eye wash, sharps kit<br>Spill kits - for fuel & liquid storage & bowser movements<br><br>Are personnel on site trained & competent to use relevant equipment, eg clean shaven, first aid at work trained

  • Is PPE suitable & sufficient for the task?<br>Is it good quality & clean?<br>Does it provide protection from adverse weather, eg waterproof?

  • Is welfare documented in the site pack and on site at planned location?<br>Are the workers aware of its location?<br>Is is clean & tidy and meet the requirements of CDM?<br>Consider - toilet facilities, drinking water in marked containers, somewhere to sit and eat, hand washing, shower & drying facilities?
