OGTR PC2 Plant Facility Checklist

Information

The Facility

Secure from entry of unauthorized personnel
Are the certification and biohazard stickers attached to all entries to the facility?
Does the facility have an anteroom?
If dealings can be disseminated through arthropods, does anteroom have strategies to prevent this?
Maintained so that it is a fully enclosed space, bound by walls, doors, windows, ceilings, including transparent sections.
Is the facility designed to prevent the entry of surface run-off water?
Are all boundaries rigid and durable?
Are drainage exits fitted with barriers (liquid traps or mesh) to prevent animals, arthropods or any plants from entering or exiting the facility?
Do other openings within facility (such as vents) have fine mesh screens to prevent entry or exit of arthropods and animals?
Does the facility have a pest control system in place?

Is work involving GM micro-organisms conducted in this facility?
Surfaces are maintained so that they continue to be smooth, impermeable to water, cleanable and resistant to damage by cleaning agents and/or disinfectants in use:<br>- walls, floors and benches<br>- furniture including seating<br>- any other surface where decontamination is likely or decontamination is required.
Open spaces between and under benches, cabinets and equipment are accessible for decontamination.
Drainage exits must be filled with liquid traps containing appropriate decontaminant or liquid waste contained and treated as waste?

Is there any equipment attached to the potable water supply?
Has a risk assessment been conducted on possible contamination of the water supply?
Has a backflow prevention device been fitted for the facility or tap (not building)?
Has the backflow prevention device been tested annually and are records available?

Are disinfectants effective against GMOs available in the facility for decontamination?
Is there appropriate labeling such as volumes and expiration dates?
Are autoclaves used for the decontamination of waste prior to disposal?
- are autoclaves calibrated annually and are records for last 5 years available?
- are autoclaves validated monthly and are records for the last 12 months available?
Is waste decontaminated at site other than facility?
Staff are trained to perform decontamination.
Soil or growth media must be decontaminated prior to re-use.
Work benches, surfaces and equipment used with GM pollen, seeds or GM micro-organisms must be decontaminated when dealings are completed.
Equipment, pots, bags, soil or soil substitutes contaminated with GM plant material, pollen, seeds or micro-organisms decontaminated before removal from the facility.
Is contaminated PPE decontaminated?

Facility doors are closed while dealings are in progress.
GM plants and materials handled to ensure containment within the facility.
GM plants and materials/trays are clearly labelled to indicate their identity.
Procedures are in place to ensure non-GMO work is not cross contaminated and procedures are documented.
In the event of transport out of the facility, is the outermost container free from contamination?
Personal protective clothing is worn while dealings are being conducted and is removed prior to leaving the facility.
Are hands decontaminated prior to leaving the facility?

Live GM plants must not be stored outside of the facility.
GMOs (such as pollen, seeds, tubers, bulbs, corms, dormant GM stems or material containing micro-organisms) stored outside of the facility are labelled (inc. biohazard symbol) and double contained.
Is all GM material including cultures of GM micro-organisms clearly labelled and sealed if in storage within the facility?
All unlabelled viable material is treated as GMO.
Fridges/Freezers/CT Rooms used for storage external to facility are labelled with biohazard symbol.
