Audit

1.0 - Previous Inspection

1.1 - Has the last inspection been reviewed?

1.2 - Are there any outstanding actions?

2.0 - General Matter

2.1 - Are the unnecessary things removed?

2.2 - Are the frequently used tools / equipments well arranged?

2.3 - Is the floor cleaned regularly and free from oil?

2.4 - Is there enough lighting in the station?

2.5 - Make sure that the emergency switch and switch board are turn off before maintenance.

2.6 - Make sure that the fuse is switch off before maintenance.

2.7 - Make sure that the signboard "Caution, maintenance work is in progress“ is place on the switch board before maintenance begins.

2.8 - Are the PPE used during working?

2.9 - Make sure that there is no one near the machines during operation start-up.

2.10 - Does the worker used earplugs while working in the kernel plant?

2.11 - Are the warning signs placed at the right location?

2.12 - Make sure that there is a pallet covered with rubber underneath the front switch board.

2.13 - Make sure that the switch board is in dry condition (not wet).

2.14 - Make sure that there is no exposed cable wire.

2.15 - Make sure that there is no unsafe act / condition.

3.0 - Tank (Hot Water Tank)

3.1 - Are the drains / floor cleaned and free from mud?

3.2 - Are the manholes closed during operation?

3.3 - Does the worker understands the works in a confined space?

3.4 - Is there any warning sign of "Beware of slippery floor" being put up?

3.5 - Is there any warning sign of "Caution, confined space" being put up?

3.6 - Make sure that the tank is labelled.

3.7 - Make sure that there is no unsafe act / condition.

4.0 - Boiler

4.1 - Make sure that there is no exposed cable wire.

4.2 - Make sure that the switch board is in dry condition (not wet).

4.3 - Are the manholes closed during operation?

4.4 - Is the steam line pipe covered with insulator?

4.5 - Make sure that the boiler is labelled.

4.6 - Is there any warning sign of "Caution, confined space" being put up for the furnace, steam drum and mud drum?

4.7 - Is there any warning sign being put up at the blow down chamber area?

4.8 - Is there any warning sign being put up at the dust collector area?

4.9 - Does the worker wears face mask while working at the racking furnace?

4.10 - Is there any negative draft during the racking furnace?

4.11 - Are the long racking tools in a good condition?

4.12 - Is the ashes dampened at the soot conveyor?

4.13 - Make sure that there is no unsafe act / condition.

5.0 - Fan & Airlock (ID Fan, FD Fan, Secondary Fan, FF Fan, Dust Bunker Airlock)

5.1 - Make sure that there is no exposed wire.

5.2 - Make sure that there is a machine guide for belting.

5.3 - Make sure that the fan and airlock are labelled.

5.4 - Make sure that the emergency switch for the fan and airlock is labelled.

5.5 - Make sure that there is no unsafe act / condition.

6.0 - Elevator (NA)

6.1 - Are the stairs and platforms cleaned regularly and free of oil?

6.2 - Make sure that there is no exposed cable wire.

6.3 - Make sure that there is a machine guide for transmission chain or coupling.

6.4 - Make sure that there is a closed cover for the elevator.

6.5 - Make sure that the elevator is labelled.

6.6 - Make sure that the emergency switch for the elevator is labelled.

6.7 - Make sure that the hand rails and pathways are free from thick dust.

6.8 - Is the floor cleaned regularly and free from kernel?

6.9 - Make sure that the spillage is disposed off or recycled back immediately.

6.10 - Make sure that there is no unsafe act / condition.

7.0 - Softener

7.1 - Is there any warning sign of "Caution, confined space" being put up at the softener?

7.2 - Make sure that the softener is labelled.

7.3 - Is the floor cleaned regularly and in dry condition?

7.4 - Make sure that there is no unsafe act / condition.

8.0 - Ribbon Conveyor( Soot CV, Fiber and Shell Conveyor)

8.1 - Are the stairs cleaned regularly and free from kernel?

8.2 - Make sure that there is no exposed wire.

8.3 - Make sure that there is a machine guide for transmission chain or coupling.

8.4 - Make sure that there is a closed cover for the Ribbon conveyor.

8.5 - Make sure that the motor is not in an oily or wet condition.

8.6 - Make sure that the Ribbon conveyor is labelled.

8.7 - Make sure that the emergency switch for the Ribbon conveyor is labelled.

8.8 - Is the floor cleaned regularly and free from kernel?

8.9 - Make sure that any spillage is disposed off or recycled back immediately.

8.10 - Make sure that there is no unsafe act / condition.

9.0 - Pump (Boiler Feed Pump, Chemical Dosing Pump)

9.1 - Is the belting guide placed on the pump?

9.2 - Make sure that there is no exposed wire.

9.3 - Make sure that the motor is not in a wet condition.

9.4 - Make sure that the motor's cover is placed securely and properly.

9.5 - Make sure that the pump is labelled.

9.6 - Make sure that the emergency switch for the pump is labelled.

9.7 - Is the rubber hose for the dosing pump placed neatly?

9.8 - Is there a cover for the chemical dosing tank?

9.9 - Make sure that there is no unsafe act / condition.

10.0 - Scrapper Bar Conveyor (Boiler Feed Conveyor)

10.1 - Are the stairs and platforms cleaned regularly?

10.2 - Make sure that there is no exposed cable wire.

10.3 - Make sure that there is a machine guide for transmission chain.

10.4 - Make sure that there is a closed cover for the scrapper bar conveyor.

10.5 - Make sure that the scrapper bar conveyor is labelled.

10.6 - Make sure that the emergency switch for the scrapper bar conveyor is labelled.

10.7 - Make sure that there is no unsafe act / condition.

11.0 - Daerator

11.1 - Is there any warning sign of "Caution, confined space" being put up at the daerator?

11.2 - Is the platform / walkway / handrail in good condition?

11.3 - Is the workplace cleaned?

11.4 - Make sure that there is no unsafe act / condition.
