Are the scaffolds properly erected & have updated green scaffold tags displayed?

Are the wastes regularly disposed off & area undergone proper housekeeping?

Are the smoke detector covers removed when no fire warden or staff is available?

Is the area properly barricaded with appropriate signages?

Are the lifts properly protected when used to transport materials?

Are the MEWPs having proper tire & under carriage protection?

Is there a Sub PTW involved?

Is there an ongoing contractor activity upon inspection?

Is there a valid Hot Works permit in place?

Is there a trained & certified fire warden available to watch over the activity?

Is the area for conducting hot work properly isolated?

Are the staff working on hot work activity wearing proper PPEs?

Is there an available gas monitoring device & properly calibrated?

Is the CS Entrant wearing proper PPEs (respirator mask, hard hat, safety shoes, hi-vest)?

Is the CS Entrant wearing a body harness attached to a rescue tripod?

Is gas monitoring being conducted at regular intervals & recorded?

Is there sufficient lighting especially when the CS is conducted at night?

Is there a CS Attendant monitoring the activity while the Entrant is inside the CS?

Are the contractors aware of the Emergency Rescue Plan for CS?

Are the contractors wearing full body harnesses & properly attached to lanyards?

Is the affected area properly isolated and barricaded with appropriate signage?

Is the work at Height activity closely monitored by a supervisor and/or HSE?

Is there an ongoing contractor activity upon inspection?

Is there a valid Excavation permit in place?

Is the excavated area properly isolated and barricaded with appropriate signage?

Are the surrounding assets (benches, running tracks, facade walls, etc.) properly protected?

Is the Emergency Vehicle Access route clear from any obstructions?

Are there any equipment placed/ parked near the excavated pit (less than 1 metre away)?

Are the contractors wearing complete & proper PPEs?
