ADMINISTRATIVE PROCEDURES

  • Does the site have the safety cylinder with the Site Safety Plan and sign in sheet?

  • Are, contractors, visitors ; Rivergum Inducted ?

  • Have the contractors signed onto the site sign in sheet?

  • Do the contractors have a white card on them for you to inspect? (phone photo is acceptable)

  • Does the contractor have a Safe Work Method Statements (SWSM) for high- risk work?

  • Has the SWMS been reviewed by a Rivergum worker?

  • Does the contractor know how to report an incident and what is a notifiable incident is?

CORRECTIVE ACTIONS

  • Are there any corrective actions that need to be undertaken ?

ELECTRICAL HAZARDS

  • Are electrical leads fit for purpose? ( not damaged, in test date)

  • Is electrical equipment RCD protected?

  • Have contractors put in place a plan to protect underground assets? i.e. Dial Before you Dig- DBYD, asset locations identified and marked

  • If there is a risk of contact with overhead electrical lines, has the risk been controlled?

  • Are leads protected from damage i.e. not placed on the ground, in walkways or road ways, or on metal structures?

  • Is electrical equipment safe from water?

EMERGENCY RESPONSE

  • Are the Emergency procedures displayed at the site? E.g. Rivergum sign or fence

  • Are first aid kits /equipment for site compliant (checked once per year, stocked as required for task and contents are within use-by dates?

  • Do the workers on site have access to a competent first aider?

ENVIROMENTAL HAZARDS

  • Are contractors preventing building materials and waste from entering storm water drains?

  • Does the site have a wash down area for painting equipment and plastering equipment?

HAZARDOUS CHEMICALS

  • Does the contractor have spill control equipment?

  • Are Safety Data Sheets available on site for the chemical being used?

  • Are the persons using the chemicals wearing the correct PPE?

  • Are the chemical storage containers correctly labelled? (unlabelled or stored in food containers)

HAZARDOUS MANUAL TASKS

  • Have hazardous manual tasks been risk assessed for the task being undertaken? E.g. Refer to the SWMS or risk assessment being used for the task

  • Have the workers been trained in the risk controls and safe lifting/ team lifting techniques?

LADDERS

  • Are ladders that are in use fit for purpose? Ladders must be compliant to AS/NZS 1892 Portable Ladders, and have a commercial load rating of >130Kg

  • Is the positioning of ladders safe and are they correctly secured?

  • Is the ladder suitable for the task? (e.g. metal ladder used for electrical work)

PERSONAL PROTECTIVE EQUIPMENT

  • Are workers wearing PPE appropriate for task (including sun protection, Hi Viz vest safety boots, etc)

  • Have Workers been trained in the safe use, clean-up and inspection of PPE?

PLANT AND EQUIPMENT

  • Is the contractor's plant fit for purpose? (check for missing or damaged guards, test and tag, generator with RCD)

  • Do earthmoving operators have the correct tickets and Licenses?

  • Do the earthmoving plant operators have a SWMS? Request to see the SWMS if they cannot produce one, then the job must stop

SCAFFOLDS

  • Does the person erecting scaffold above 4m have the high-risk scaffold ticket? They must stop work immediately - report this to your manager ASAP

  • Is the scaffold fit for purpose? E.g. All items are present and secured, no evidence of tampering, has a current Scaff tag and a handover certificate

  • Has the scaffold been inspected by a competent person within the last 30 days?

  • Is the scaffold being used for its intended purpose (check the handover certificate), for example, -Safe Work Load (SWL) exceeded? (tools, stored materials, number of persons)

WORK ENVIRONMENT

  • Are site toilets fit for purpose - E.g. clean and tidy, have soap and water for washing hands and toilet paper ?

  • Does the site bin have room for more rubbish?

  • Is housekeeping acceptable? E.g. are walkways free of debris, public footpaths free of trips and slips, contractors are cleaning up as they go.

WORKING AT HEIGHTS

  • Are fall risks that could cause an injury safely controlled? (including voids, pits, and trenches)

  • Are all open edges protected? E.g. edges are protected from risk of falls with barricades or scaffold

  • Are contractors performing work at heights as per their SWMS? Request to see the SWMS if they cannot produce one, then the job must stop
