EXCAVATION

  • Has Digger's Hotline been called (1-800-242-8511; JSS #5983)

  • Underground electrical lines marked

  • Underground gas lines marked

  • Excavation daily checklist completed

  • Adequate benching, sloping, and/or trenching in place

  • Access and egress means provided

  • Spoil set back at least 2 feet

  • Is the excavation more than 4ft if so, is it adequately protected and has a confined space permit been filled out

LADDERS

  • Ladders in good condition

  • Side rails extend 36 inches above landing

  • Proper for job and secure

  • Step ladders fully open when in use

  • Placement of ladder safe

SCAFFOLDING

  • Is there a scaffold tag and has it been inspected

  • All scaffolds plumb and level

  • Are adequate sills and base plates being used

  • Are metal scaffold planks installed correctly with none extending past their end supports

  • Are wooden scaffold planks installed past their end supports no less than 6 inches and no more than 12 inches

  • Are guardrails including top and mid rail in place

  • To protect from falling objects, are toe boards or area barricaded below

  • Is fall protection being used in areas where required

  • Is their a safe access ladder, steps or equivalent means for safe access to the scaffold

  • Is the scaffold rated for the proper weight capacity

FIRE PREVENTION

  • Are hot work permits in order

  • Fire extinguishers immediately available and inspection tags current

  • Is a fire watch monitoring the area

  • Propane tanks - barricade installed - labeled -( flammable,no smoking, fuel type)

  • Compressed gas cylinders secured- upright- protected from damage

  • Cutting torch (tanks secured, hoses, etc.)

PPE

  • Hard hats worn

  • Eye protection being worn and adequate for the work activity

  • Face shields being used where required

  • Gloves being worn and adequate for the work activity

  • Welding hoods being worn in conjunction with safety glasses

  • Safety harness and lanyards utilized with adequate anchorage points

  • Hearing protection

  • Respirators and masks utilized, when needed

  • High visible clothing in use, when needed

CONFINED SPACES

  • Proper permits filled out

  • Permits posted at entry

  • Attendants posted at openings

  • Atmosphere tested and results documented

  • Entry Supervisor has classified confined space and authorized entry

  • Everyone has been properly trained

SUBCONTRACTORS

  • Subs wearing proper PPE
