Maintenance of Store and Fixtures

  • Are display racks, shelves, and fixtures sturdy and appealing? (not showing signs of rust or extreme wear)

  • Are there broken hangers, display fixtures, or mannequins on the selling floor?

  • Are mirrors clean and free from fingerprints, smudges, stains, cracks?

  • Are seating areas well-maintained (e.g., no wear and tear or stains on cushions)?

  • Are air conditioning units functioning properly and clean?

Storage/Back-of-Room (if accessible)

  • Are lights in good working condition and adequately illuminating the stocks?

  • Is the storage or stockroom area tidy and organized?

  • Are personal belongings stored properly and out of sight? (utilize lockers only)

  • Is the pantry clean and organized? (if applicable only)

  • Are empty boxes or packaging materials cleared away or stored away neatly?

  • Are the walkways and aisles excessively blocked by stocks?

  • Are the fire sprinkler blocked by shelves or products?

Employees Compliance

  • All staffs are properly groomed (following the respective dress code of location)

  • State what was not in compliance (e.g: using cap, hair messy)

  • Staff members are focused on the job, you see them attending to their task instead of chatting away

  • Staff members are seen using their phones at the selling floor.

  • Staff members are seen chewing, eating, or sitting on the bench/floor of the selling floor.

  • If you noticed something unpleasant regarding staffs behaviour, please include your observation

Customer Centric

  • Did staff acknowledge customer within 1 minute of entering the store

  • Did all the staff member appear to be ready to serve you/customers?

  • Please state the poor behaviour from your observation.

  • When you or customers were greeted, what was said?

  • Did the staff member smile and make eye contact?

  • Did all OTHER associates/team members you encountered or passed throughout the store smile and greet you? (even if they were performing their own tasks)

  • All associates/team members are courteous?

  • Did the staff member try to assist you?

  • If you, or the customer asked to be left to browse through, did the staff member gave assurance that they will be nearby if you/customers need their assistance?

  • Did the staff member suggest an item?

  • Did the staff member inform you of any promotions/discounts going on?

  • Did the staff member give you/customers his/her undivided attention? (not playing with phones, or attending to their tasks first, etc)

  • You can include a brief detail of your interaction with the sales floor associate. You may ask about a product and observe the staff's response. (e.g: Asked staff about a ladies footwear, he was able to suggest the best option for my wide feet, he made a point to ask me what am I most comfortable with.) - min. of 3-4 sentences required.

  • Did you observe team work between staff members call for additional help (to manage another cash register or assist customer, if the store was busy?)

  • Was greeting/acknowledgement heard from the cashier the moment customers reached the counter?

  • Did the cashier ask if you would like to add anything else?

  • How long did your transaction take from the time you reached the counter until you received your receipt? (if applicable)

  • Did the cashier thank you/customers at the end of your transaction?

  • Did the associate/staff member provide a pleasant closing comment (“Have a good day,” “Thanks for shopping with us,” “Thank you,” etc.) before you/customers exit the store even without a purchase?

  • Did any associates/team members provide above-and-beyond service?

  • Please state his/her name and department and describe the above-and-beyond behavior.
