Shirt/Jacket

  • Free of tears or rips any size will reduce the production level?

  • Free of worn areas i.e. elbows, shoulder or back areas?

  • Sleeves Velcro in good shape and not gummed up or torn?

  • Zipper Velcro cover in good shape and not gummed up or torn?

  • Free of oil or grease stains?

  • Is the Flame Retardant labels readable?

  • Free of any repairs? If so was the repair in accordance with manufacturer's recommendations?

Pants

  • Free of tears or rips any size will reduce the production level?

  • Free of worn areas in the knee or butt areas?

  • Free ofoil or grease stains?

  • Free of any repairs? If so was the repair in accordance with manufacturer's recommendations?

  • Fasteners and zippers/buttons in good repair and not damaged in any way?

  • Is the Flame Retardant labels readable?

Faceshield

  • Is the face shield in good Condition. Check the face shield visibility, inspect for excessive scratching. If visibility is impaired or the shield is cracked or damaged remove from use (replace with appropriate lens)

  • Does the face shield manually adjust: mounting mechanism (adapters) to ensure wing nuts are firmly in place, do not over tighten or the shield won’t be able to operate correctly

  • Is the adapters free of cracks or damage, ensure fingers are locked into hard hat slots

  • Is the hard hat slots in good condition? and inspect for cracks that could allow the adapter to slip out

  • Is the nuts and bolts fastening a lower chin guard (double crown) are can be tighten?

Helment

  • Date of Manufacture:

  • Date put into Service: (If unknown, Use date of manufacture)

  • Is the hard hat/helmet have a Class E Designation? If not notify your supervisor immediately

  • Is the helmet free of any dents, cracks, tears, gouges or holes in the shell in any way?

  • Has the inside suspension been replaced in the last 12 months?

  • Is the manufacturer's tag attached and readable?

  • Is the suspension free of any frayed or cut straps?

  • Is the suspension system free of cracks or tears (which keeps the shell from touching the wearer's head) or loss of flexibility?

  • Is the helmet free of brittleness or dullness of the shell (it may look chalky)?Replace

  • When the helmet is compressed does the helmet bounce back into normal shape? Compress the shell from the sides using both hands to check for elasticity. The shell should bounce back and be as elastic as a new one

  • Now Inspected by Out side Company every 6 months
