Where relevant, working at heights is part of induction

People working at heights have been given information, instruction and training such as Working at Heights

Workers are supervised to ensure that safe work practices are in place

Are all materials secured from movement

Work areas are clear of protruding objects, water, vehicles and people

All work areas are free from obstructions

Is edge protection erected to comply with regulations where a person could fall 2m or more.

Walkways, corridors and stairs are free from obstructions

Has safe access been provided e.g. Stair or ladder access

Hand rails on stairs are secure and steps are well maintained

Ladders are in good condition and are secure and fixed firmly in place

High ladders have fall back protection

Mechanical lifts are safe

Fall arrest systems, such as harnesses, are in place

If fall arrest systems in place do anchor points comply with relevant Australian Standards

Is PPE inspected and maintained and all fall restraint register established

Safer, alternative ways to do the work have been considered

Has a risk assessment been carried out before commencement of any work at heights

Potential existing hazards have been identified

Practical steps have been taken to prevent falls

Is there a permit system in place for working at heights

Is there a rescue plan in place

Are any holes or openings in any floor appropriately covered have appropriate signage "Danger Hole Beneath" and/or is protected using guard rails

Risk Rating
