POWERED PLANT & EQUIPMENT

  • Is all Power Plant (I.e bob cat, excavator, crane etc.) logged into plant register?

  • Have daily pre-start checks been filled out in Powered Plant log books? (Random checks)

  • Do plant operators hold appropriate High Risk license or competency?

  • Is appropriate PPE being used by plant operator?

  • Is mobile plant being operated well clear of overhead power lines?

  • Are lifting slings, chains, shackles certified?

  • Are appropriate machine guards in place?

  • Are speed controls implemented and being observed?

  • Are seat restraints being used correctly?

EXCAVATIONS

  • Are excavations which have a potential to collapse inward, benched or battered?

  • Are excavations barricaded to prevent personnel falling in?

  • Is plant and excavated soil kept at a safe distance from the edge?

  • Are barricade presented in good condition and fit for purpose?

  • Do open excavations have a safe means of access/egress?

WORKING AT HEIGHT EQUIPMENT

SCAFFOLD

  • Is the scaffold tagged as certified safe for use at the entry points?

  • Are feet located on a stable foundation?

  • Guard rails and toe boards in place and secured

  • Is the working platform clean and free from obstructions?<br>

  • Are all legs grounded by the base plates?

LADDERS

  • Ladders extend a minimum of 1m above connection point and are secured (i.e. tied down)

  • Ladders are maintained in good condition

  • Ladders have legs correctly spaced and locked/tied off

  • Top step of step ladders are not being used as a working platform

OPEN FLOOR PENETRATIONS

  • Holes or opening in floors are barricaded with danger signage

GUARD RAILS

  • Are Guard rails in place where a person can fall 2m or more from a stair, landing or fixed platform

  • Guard rails are adequately secured

HAZARDOUS SUBSTANCES AND DANGEROUS GOODS

  • Is a storage facility established which is well ventilated, away from potential ignition sources and appropriately labelled (e.g. HAZCHEM)

  • Chemicals and dangerous goods are appropriately labelled with their contents

  • Are gas bottles secured?

  • Are all substances stored in sealed containers?

  • Incompatible chemicals are segregated

  • MSDS file available at the storage point which identifies appropriate chemicals

  • Is there evidence of spills or leakage- (if so; attend to spill and dispose of waste responsibly)

PERSONAL PROTECTIVE EQUIPMENT (PPE)

  • Mandatory PPE is being worn by subcontractors (Hard Hat, High Vis, Steel Cap footwear)

  • Task specific PPE is being appropriately used by personnel

ENVIRONMENTAL

  • Powered plant emitting black exhaust is scheduled for maintenance

  • Excessively noisy tasks are not undertaken in the early morning (disturbing local community)

  • Vibration dampening equipment is installed where necessary

  • Dust generated by concrete/brick cutting is being minimised (i.e. wet cut, vacuum, dustless cutters etc)<br>

  • Plant is being switched off when not in use (i.e. not left idling for prolonged periods)

  • Spill kits are available on site and adequately stocked

  • Water supplies turned off or inspected for leakage

  • The site has a designated re-fuelling area

  • Designated concrete wash out points implemented where practicable
