Preparation to Grout

Have all tendons been checked for clear passage of air?

Do any tendons require unblocking/drilling?
Grout Mixture

Is Premix Grout being used? (Auscon PT Cement/Crosbe)

Note the name of the grout additive being used (If using GP Cement) and the quantity placed in an 8 bag mix.

Is the water to cement ratio being used in the range according to product data sheet? (8-9 Litres of water per 20kgs cement) or (7.1 Litres per 17kg bag of Auscon PT Grout)

Was the water and additive measured into the mix?

What type of duct is in use for the tendon?
- Flat 70mm
- Flat 90mm
- Round

Has the slab been checked for looped tendons?

Has the Bucket Operator confirmed quality grout expelled for all tendons?
Grouting Result

Has the perimeter of the slab been checked for leaks or sprayed grout?

Are all tendons fully grouted?
- Yes
- If No, type in tendons numbers and why not grouted

Is follow up required before signing the slab off as grouted?
- No
- If Yes, send incomplete form to construction Manager for review

Grout Pump Operator - I confirm the slab has been fully grouted as noted above.

Bucket Operator - I confirm the slab has been fully grouted as noted above.

Nozzle Operator - I confirm the slab has been fully grouted as noted above.

SRG Leading Hand - I confirm the post-tensioning slab has been fully grouted.
