• I acknowledge that I have received a new helmet and agree that I will use this new helmet from now on, as my head protection PPE, completely replacing my old hard hat that will be put out of service indefinitely. I agree to use the new helmet as per requirements, always secured with the chin strap and for working purposes only. If I modify it, paint or draw on it I will be responsible for replacement.
