New Hire Checklist

  • Front page of application to be filled out by employee

  • W4 to be filled out by employee

  • G-4 ( State withholding) form to be filled out by employee

  • Line "H" should equal the total of lines "A" - "G"

  • Box 5 ( bottom half of form); should be the same number as line "H"

  • Signatures and employee information is filled out correctly

  • G-4 pg.2. Employee to fill out

  • Make sure #7 has the same letter as the option picked at the top

  • Form signed, and employee information is entered correctly

  • Fill out I-9 form. You can assist the employee with this form if needed. <br>Make sure you sign at the bottom where it ask about assistance

  • Is the USCIS number 13 digits?

  • I-9 pg. 2: Page 2, you only need to fill out either A or B.<br>You don't need both.

  • Go over safety policy. Make sure employee understands, and takes that copy with him.

  • Employee signs the Safety Policy Acknowledgement form

  • Employee signs the workers comp.and injury reporting agreement

  • Employee signs form submitting to drug testing

  • Fill out insurance paperwork (FULL TIME ONLY)

  • Make photo copies of S.S. card & Perm. Resident Card

After paperwork is filled out

  • Scan signed forms and documents, and email them to Susan Newsom.<br>(Texas area include justin Mathews). (Atlanta & eastern areas, include Mike Edwards)

  • Mail original copies of the application to <br>2160 Kingston Ct. Ste. I Marietta, GA 30067

  • Let Susan Newsom know when you are starting the employee, via phone or email

  • GET EMPLOYEE DRUG TESTED THAT DAY!<br>(Contact Mike Edwards or Justin Mathews for drug test forms)
