EHS POLICY 119: WALKING AND WORKING SURFACES POLICY

  • Any working surface above 4' from the ground must be protected by a work platform with guard rails at least 42" above platform, have an intermediate mid-rail and toe-board or personal fall arrest system

  • Fall arrest system must be utilized when working above 4' on an unguarded surface

  • Anchorage used for fall arrest system must be capable of supporting at least 5,000lbs.

  • All walking and working surfaces are clear of debris and trip hazards and/or trip hazards have been identified

EHS POLICY 120: LADDER SAFETY

  • No wooden ladders allowed onsite

  • Metal/fiberglass ladders must have a warning label for electrical caution

  • Ends/feet of ladder must be fitted with insulated non-slip material

  • Extension ladders must extend 3' past the landing if used to access another working surface

  • 3 points of contact being utilized by workers while on ladders

  • Fixed ladders are to be inspected annually and fill out a Fixed Ladder Annual Inspection Checklist

  • Portable ladders are to be inspected monthly and fill out a Portable Ladder Monthly Inspection Checklist

EHS POLICY 122: PRE-JOB SAFETY ASSESSMENT

  • Pre-PJSA safety tour of site conducted

HAULING/LOAD OUT PROCEDURES

  • Haul trucks stops at gate on level surface, driver walks pad to monitor LEL levels

  • Facility vapor recovery system utilized while loading

  • Method 27 and pressure/vacuum tests within 12 months of certification

FLOWBACK EQUIPMENT

  • Wellhead is equipped with ESD valves with a remote shut off control panel placed NLT 50' from wellhead along egress path

  • All flow lines are rated at 15,000 psi

  • All manifolds are rated at 15,000psi

  • Certification bands present on all manifolds and flow lines, not exceeding 12 months from last certification

  • All separation and well testing vessels ASME code stamped and rated for operating pressure

  • All connections are flanged or hammer unions, no threaded connections presents, with exception of connection to CHK sand trap

  • Cement blocks placed over flow lines, at a minimum, every 50' and at each change in direction

  • Flow lines secured with steel cable whip checks

  • Minimum of 1 fixed LEL monitor placed at the Flowback Tanks

  • Flowback tanks grounded

  • Primary flare stack placed 35' from Flowback equipment, secondary placed 15' from Flowback equipment

  • Wind socks deployed on Flowback tanks

FLOWBACK OPERATIONS

  • No more than 2 wellheads in Flowback at any given time

  • CHK Rep on location during all Flowback operations

  • Double block and bleed utilized during blowdown operations

  • Entrance gate closed or roped off with check in sign
