Do we have documented evidence of all personnel that complete welding and oxy-cutting activities as being appropriately qualified?

Are Hot Work permits being used for all welding and oxy-cutting activities done outside a designated hot work area (i.e. an assessed welding bay)?

Do all personnel conducting welding and oxy-cutting activities have the appropriate attire and PPE on i.e. flame retardant clothes, gloves, aprons, welding shield etc.?

Is the areas where welding and oxy cutting undertaken adequately ventilated to ensure no build-up of fumes?

Is a fire extinguisher always nearby any welding or oxy-cutting activities?

Is all welding and oxy-cutting equipment in good condition and is checked regularly?

Are oxygen and acetylene gas cylinders not in use (i.e. not on a trolley) stored at least 3 metres apart. In a well-ventilated area and are restrained in an upright position?

Is there a Danger sign above flammable gas storage areas to indicate No smoking or Ignition sources”?

Are fire lighters provided to light oxy-acetylene torches with i.e. no Matches or lighters)?

Are flashback arrestors fitted to oxy-acetylene kits?

Are welding electrodes stored correctly i.e. Low hydrogen electrodes stored in a controlled hot box?

Are welding blankets available and welding screens in place to protect others in the workplace and potential fire risks when welding/cutting is done?
