1. Job Briefing

1.2 Has the scope and the extent of the services to be performed been discussed with and understood by each work group member?

1.4 Are appropriate tasks being assigned to individual Operators or persons in control of a task?

2. Worksite Hazard Management

3. Safe Work Environment

(3 - 3.42) 3. Have the applicable hazard(s) been identified and relevant control measure(s) been implemented, and the level of planning been satisfied to eliminate / minimise all worksite hazards? (Reference:- Safe system of work, and MUTCD)

3.2 Has a Worksite Emergency Plan (Worksite Safety Management Plan) been established, available to all persons at the worksite, and being complied with (if applicable)? (Including clear rescue path identified, evacuation point established.)

3.3 When required, has an appropriate sized “No Go Area” been identified and delineated? (With barriers and signage in place.)

3.11 When working on or working near energised network, are protective barriers in place on all secondary points of contact that are within reach, prior to the commencement of work (including insulating mats and covers)?

3.15 If suspected contaminated materials (e.g. Asbestos Contaminated Materials (ACM)) are located / identified: Are appropriate measures taken to limit exposure? Are appropriate storage and disposal methods adopted? Is there compliance to current safe system of work?

3.18 Has the applicable traffic management (for vehicular and pedestrian) control measures been established (for example correct placement of signs, cones and barriers) for each work task and has the levels of planning been satisfied (for example hand sketch, nominated WMS, Traffic Management Plan)?

3.24 Do Operators demonstrate understanding / knowledge of confined spaces, their identification (limitations and conditions for entry to undertake work)?

3.39 Is a two-person crew in attendance at a Customer’s premises to undertake all disconnection / de-energisation for non-payment and reconnection for non-payment?

3.40 Is the Safety Observer at the Customer’s premises while disconnection / de-energisation for non-payment and reconnection for non-payment is occurring; identifying, taking the necessary actions and warning the Operator undertaking the work of developing situational hazards and deteriorating security at the worksite?

3.41 When required, do Operators and persons in control of a task understand / demonstrate knowledge of approach distances? (Minimum safe approach distances maintained.)

4. Personal Protective Equipment, Tools, Vehicles and Plant

(4-4.36) 4. Are Personal Protective Equipment (PPE), Clothing, Tools, Equipment and Vehicles maintained in a serviceable condition, stored and utilised in the correct manner and where applicable the next test due date attached? (Reference:- Safe system of work)

7. Environmental Management

7.76 Do Operators demonstrate understanding / knowledge and identification of animal and plant diseases, pests and other biosecurity matter (for example and not limited to Invasive Plants, Fire Ants, pathogens and exotic insects) potentially located in the region(s) they are operating and comply with the measures taken to control the spread of animal and plant diseases, pests and other biosecurity matter issued by the Authorities in control for their containment?

16. Document and Record Management

(16-16.71) 16. All standards, specifications, documents, records and completed forms established as part of management of the services provided, completed, filed and available (all records well ordered)? (Reference:- Sections 9, 10 and 13 of WCS57)

16.1 Are all Work Group members aware of contents of appropriate standards, specifications, forms / documents and are they available at Worksite and in a serviceable condition?

61. Service Delivery Standards

61.14 Is the Service Provider or their nominated Operator providing Energy Queensland with full details of all planned outages to electricity supply at Customer’s installation that does not require switching of the network within the notification time frames; providing the data in an electronic format through the prescribed interfaces with Energy Queensland corporate systems (SANS System or EPP System)?

61.15 Is the Service Provider or their nominated Operator providing Energy Queensland with full details of all completed works (jobs) and planned outages to Customer(s) electricity supply that does not require switching of the network within the notification time frames of the planned outage occurring and work being completed; providing the data in an electronic format through the prescribed interfaces with Energy Queensland corporate systems (SANS System or EPP System)?
70. Service Delivery Standards – Customer’s Installation

(70-70.7) 70. Is Customer treated in compliance with the Energy Queensland Code of Conduct and Customer Care Requirements? (Reference: - Section 9 of WCS57).
