INTRODUCTION

  • 1. Identify the work area to be inspected, in accordance with the Hazard Inspection Matrix. IRC-WHS-SP-006.
    2. Utilise this checklist to conduct the hazard inspection of the work area. The team conducting the hazard inspection should include a WHS advisor, WHSR and supervisors/team leader of the work area.
    3. Document findings including any observations and 5 key recommendations
    4. Forward the completed checklist and report to safety.reports@isaac.qld.gov.au
    5. WHS team will upload the checklist and report into LOGOSS and will allocate actions to responsible person/s.

OFFICE

  • Is the office IRC Badged?

  • Is the office able to be secured when vacant?

  • Is there an allocated delivery area for packages and is it signed?

  • Are the work areas/ workstations kept clean and walkways clear?

  • Is the office fitted with air-conditioning and set at an acceptable level?

  • Are work areas sufficiently lit?

  • Is there a visitor’s sign in register available and are visitors provided with visitor’s safety information? Are they escorted at all times?

NOTICEBOARDS - SAFETY

  • Does the noticeboard display current documents?

  • Is the WHS & Rehab Policy displayed and is it current version?

  • Are WHS obligation statements displayed, are they current version?

  • Is Safety contact names (wardens, first aiders, RRTWC, Safety Advisors, WHS reps) displayed, is this current version?

EMERGENCY MANAGEMENT

  • Is there evacuation diagrams located around the office?

  • Are the emergency exits signed?

  • Is there an emergency alarm in the office?

  • Is there emergency assembly areas allocated?

  • Are there first aid kits available, current, and clearly signed?

  • Is there firefighting equipment available (hose reel, extinguishers, fire blankets) and free from obstruction?

  • Is there an eye wash station available?

SMOKE AREA

  • Is there a designated smoking area?

  • Is the smoking area signed and has a receptacle?

ELECTRICAL SAFETY

  • Are compliant power boards used? i.e.; safety switch on power board, NO double adaptors

  • Is there clear access to the main power board/s - is it secured to prevent unauthorised access?

  • Are there 'Out of service'/ 'Danger'/'Information' tags available?

AMENITIES

  • Are toilets and shower facilities provided, clean and tidy?

  • Are the female toilets fitted with a sanitary receptacle

  • Is hand drying facilities available?

  • Are dining facilities available and clean?

  • Is potable drinking water available?

SIGNAGE

  • Are relevant safety signs displayed near equipment and at entry to site?(e.g.; first aid, amenities, hearing protection)

  • Are the signs in good condition and appropriate?

  • Are frequently used items within easy access?

  • Are there mechanical aids to assist with manual handling?

  • Are heavy item stored at waist height?

  • Are stepladders or safe steps available to access items stored on high shelves?

HAZARDOUS SUBSTANCE STORAGE

  • Is there a register of hazardous substances?

  • Is compressed gas secured, stored according to its classification & segregated from ignition sources?

  • Are all the chemicals stored correctly, signed and bunded?

  • Are all of the chemical containers labelled?

  • Is there a spill kit available (appropriate to clean up) & stocked?

  • Are the Safety Data Sheets (SDS) current (within 5 years), located where hazardous substances stored and used?

HOUSEKEEPING & ACCESS

  • Are rubbish bins provided and is waste removed regularly?

  • Are all floor surfaces in good condition (e.g.; even and undamaged, spills cleaned up)?

  • Are ramps and steps clearly marked?

  • Are railings in place for stairs and high areas?

  • Are pedestrian walkways kept clear?

TRAFFIC MANAGEMENT, VEHICLES & PARKING AREAS

  • Is parking available, demarcated, well-lit and clearly signed?

  • Are disabled parking bays provided, are they signed?

  • Are there any pool vehicles? Do they have a prestart inspection book and well maintained?

  • Is the traffic flow for worksite identified and signed (inclusive of forklift operating areas)?

  • Is there designated pedestrian walkways identified?

PLANT & EQUIPMENT

  • Is equipment visually inspected (e.g.; pre - starts available and completed) and in good working order?

  • Does the forklift leave site? Does it have appropriate registration?

  • Is the forklift fitted with warning lights and reversing beeper?

  • Are appropriate guards fitted on all plant & equipment?

HAND & POWER TOOLS

  • Are tools in good condition?

  • Are electrical tools test and tagged and used/ stored appropriately?

NOISE

  • Has a noise survey been undertaken - date?

  • Has steps been taken to minimise exposure to noisy areas?

  • Is hearing protection available & are there signs indicating to wear hearing protection?

LIFTING EQUIPMENT

  • Is there an overhead crane- if yes (staff trained, is it registered with WHSQ and is there records/register of inspection)?

  • Is the SWL displayed?

  • Is all lifting gear within inspection date and stored correctly?

  • Are lifting hooks fitted with safety latches?

COMPRESSED AIR

  • Is there compress air tank located on site - if yes (Is it registered with WHSQ and has a records/register of inspection)?

  • Are all air lines clearly marked with correct colour coding and fittings fit for purpose?

OPEN TANKS

  • Are tanks fitted with barriers /rails to prevent entry?

  • Are tanks fitted with lifesaving equipment e.g.; ring?

  • If entry is required to tanks, is there SWMS and confined space permit completed?

WORKING ALONE OR IN ISOLATION

  • Does the work site require a person to work in isolation?

  • Has a duress alarm been provided and worker given instruction?

  • Is there a process in place for communication when workers are to work alone?

HAZARDOUS HIGH RISK WORK

  • Are there SWMS's, signed, registers, rescue plans, permits for any of the following high risk work?

  • Work at Heights

  • Confined Space

  • Welding & Cutting

  • Excavation & Trenching

  • Other high risk work e.g.; concrete pumping, working around powerlines etc.

CONSTRUCTION SITES

  • Is there a sign identifying who is in control of the workplace in accordance with procedures?

  • Is there a Construction Safety Plan and prestart/ toolbox talks available?

  • Are there SWMS's and JSA's available for the work?

  • Is there appropriate barricading/fencing around the construction site?

  • Are all workers signed onto the plan and the SWMS's for work being undertaken?

  • Is there a traffic management plan in place?

STAFF ENGAGEMENT

  • Are the workers aware reporting? What & how to report? (e.g.; incidents, damage, theft, hazards)

  • Are the workers aware of whom the first aider is?

  • Are workers aware of means of raising the alarm in the event of an emergency onsite?

  • Are the workers aware of emergency procedures relating to fire, medical, personal & bomb?

  • Are the workers aware of how to use the Isolation & Tag out system?

  • Do the workers have appropriate PPE to perform functions of their role, if not who do you report this to?

  • Are the workers aware of communication systems at the site?
