  • Is the driver of the vehicles declared competent, licensed or authorised to use the vehicle? Check for a valid driver’s license? (Vehicle / forklift / truck / tankers etc.)

  • Does the vehicle have a valid license? Check for suitability e.g. Transportation of dangerous goods, Food transportation

  • Is the vehicle equipped with the correct tram card for the product being transported? (Flammable, no smoking etc.)

  • Are the driver taking regular breaks when traveling long distances? (If applicable ask the driver)

  • Are the safest route taken to reach your destination? (If applicable ask the driver)

  • Are the vehicle inspected before use? (Check pre use inspection or ask the driver)

  • Are the driver obeying traffic rules? (Speed, road signs and road markings etc.)

  • Are chock-blocks used behind the wheels when a vehicle is parked at the plant? (Two chock-blocks to be used)

  • Are drivers and passengers wearing their safety belts?

  • Are drivers keeping a safe distance from the vehicle in front?

  • Are first aid boxes available in vehicles that travel long distances and travels off-site?

  • Are vehicles fitted with additional safety requirements? (E.g. warning triangle, fire extinguisher and check for emergency contact numbers).

  • Comments: Where deviations have been noted, indicate the corrective action taken. Also report the matter to the supervisor
