Did you identify any non-compliance with the not negotiable requirements set out in Clause 5 of the Safety Management plan?<br><br>The not negotiable requirements that must be set out as mandatory requirements in the procedures and guidelines referred to above are:<br>a. Unless necessary for the performance of the work, the Patrol must physically hold the keys to the vehicle.<br>b. Unless necessary for the performance of the work, the Patrol must always ensure that the vehicle handbrake is on and the vehicle is in neutral/park.<br>c. Members should only be allowed to remain in the vehicle while the Patrol is working on it if it is unsafe or unreasonable to direct the Members to leave the vehicle in the following circumstances:<br> i. The member has a number of young children and there is no appropriate barrier that they can stand behind while on the side of a road; or<br> ii. A member is in a wheelchair; or<br> iii. Members are elderly and it is a very hot day with no appropriate shade or an area to sit down is available.<br><br>

Did you identify any non-compliance with the not negotiable requirements set out in Clause 8 of the Safety Management plan?<br>(a) While it is recognised that sometimes a vehicle might start while a Patrol is working underneath the vehicle to start it, a Patrol must never start a vehicle and then go underneath it to work.<br>(b) A Patrol can only work underneath if:<br>i. Vehicle stands have been put in place;<br>ii. At least 2 of the vehicles's wheels have been chocked;<br>iii. The handbrake of the vehicle is on;<br>iv. The member is outside of the vehicle and well away from it, or if necessary for the member to be in the vehicle the Patrol has given very clear instructions to them explaining that they will be underneath the vehicle and what the member can and cannot do, and is certain that the member understands those instructions; and<br>v. Use the correct vehicle jack points when jacking the vehicle. <br>

Overall assessment of the Patrols risk assessment (tick the box that best describes the Patrols assessment of the work site).

Score: 1 Description: Very good. The work site was well set up and managed. There were no health and safety risks that had not been identified and controls put in place for. There was no non-compliance with any of the not negotiable requirements. The Patrol could describe the risk assessment processes they went through for their drive-by assessment and the way the work site had been set up. The Patrol could describe the controls that were supposed to be in place to manage the risks they had identify, and demonstrate how they were working in accordance with those controls.

Score: 2 Description: Acceptable. The worksite was reasonably well set up and managed. There were health and safety risks that had not been identified, or controls had not been put in place for, but there were minor risks and did not present any serious injury threat. There was no non-compliance with any of the not negotiable requirements. The Patrol could describe the risk assessment processes they went through for their drive-by assessment and the way the worksite had been set up. The Patrol could describe the controls that were supposed to be in place to manage the risks they had identify, but there were examples where they were not working in accordance with those controls.

There were health and safety risks that had not been identified, or controls had not been put in place for, including risks that did present a potential serious injury threat. There was no non-compliance with any of the not negotiable requirements. It was difficult for the Patrol to describe the risk assessment processes they went through for their drive-by assessment and the way the worksite had been set up. It was difficult for the Patrol to describe the controls that were supposed to be in place to manage the risks they had identified. There were examples where the Patrol were not working in accordance with generally acceptable controls.
