Customer Satisfaction and Installation Checklist

  • DSL/Internet

  • Modem is wired and setup

  • Router is setup (if router is available at time of install)

  • Internet test performed; speed results are within range of service package ordered

  • Educate customer on use of internet and answer any questions he/she may have

  • (If Applicable) The technician and I have agreed to the placement/installation of any and all new wiring that was performed to facilitate the delivery of the services I have requested. I understand, acknowledge and agree that anything outside of All West’s Basic Install Policy (Phone ILEC: anything past the demarc, Phone CLEC: move or rewire one jack, TV/Cable: move or rewire up to two jacks, ALL SERVICES: includes going through walls, but not fishing walls) will be billed at rate of $25 per additional jack or $90/hour, whichever is applicable, of which charges I agree to pay. I also acknowledge that I have been given the option of performing the work myself, hiring my own electrician,<br>or having All West perform the work. I have elected to have All West perform the work.

  • Wall fishing/punching performed, jacks secured to wall/baseboard; customer requested
