Procedure

  • Is there a written and approved site procedure for Light Vehicles and Traffic Management?

  • Has it been communicated to the employees ?

  • Has it been communicated to the contractors?

Company driving permit

  • Is there a head of department determine who needs a license?

  • Is the site induction conducted to be allowed to drive non or off site with a company vehicle?

  • Is the site driving permit provided having passed a practical and theory driving test?

  • Are driving permits issued annually with the induction?

  • Do all drivers have a valid DRC, Zambia or International driving license?

General rules for authorized drivers

  • No unauthorized use of vehicle allowed?

  • Do drivers know their responsible for the vehicle in his care and will be held responsible for any damage to the vehicle?<br><br><br><br>

  • Is the driver responsible for the passengers in his or her vehicle?

  • Are drivers aware no unauthorized personnel allowed in the company vehicle?

  • Are driver aware they are not allowed to drive under the influence of alcohol or illicit drugs?

Seatbelts

  • Are seatbelts being worn at all times including real seats when traveling in a vehicle?

  • Is the driver responsibility to make sure that all passengers are wearing seatbelts before the vehicle is driven away ?

  • If seatbelt is not working, no passenger allowed at in that seat?

  • Are people not traveling while standing in the tray of the pickup?

Vehicle requirements

  • Are vehicle driven with their headlights on at all times on site?

  • Are vehicles carrying full charged fire extinguisher, warning triangles, spare tyre?

  • Any vehicle going into mining areas equipped with a two way radio tuned to the mine operations frequency or a handheld radio, a green flashing light or other colors otherwise approved, flag, reflective stripes on front and back of the vehicle

  • Are private or contractor vehicle used on site inspected by Engineering department prior to being used on site?

  • Are visiting vehicle that are not inspected being escorted on the mine site by their sponsors at all time?

Pre-start checks

  • Before operating vehicle do drivers conduct pre start check and fill the prep start checklist?

  • Do these pre start checklists submitted to light vehicle maintenance section in weekly basis?

Light vehicle maintenance

  • Is there schedule for the systematic inspection and testing of all comply light vehicles?

Exclusion and Separation

  • Are the following areas restricted for entry and only be entered by employees who have been trained and authorized to do so; Mining areas, heavy vehicle workshop, construction areas, areas sign boarded accordingly and any coned or danger taped areas?

  • Are bicycles and motorbikes not allowed in the processing plant and mining areas?

  • Do drivers of motorbikes and their passengers wear helmets?

Speed limit and parking

  • Are speed limits set for each of the work areas and roads, and have these been clearly signposted?

  • Are vehicles parked in a manner that not obstruct the flow of traffic or cause a hazard to other road users or pedestrians?

  • Do drivers not leave the unattended vehicle idling at any time?

  • Are keys not left in the ignition?

  • Is the vehicle left in the first gear once the engine has been switched off?

  • Are vehicles not parked closer than 50m to any heavy equipment?

  • Are vehicles not parked or stop to disembark passengers within 10m of an intersection or in the middle of the road?

  • No passengers jump off or exist from a moving vehicle?

Overtaking, following distance, breakdown and recovery

  • Are light vehicles not overtaking moving heavy vehicles such as haul truck, water carts and loaders unless on the the main road?

  • Do light vehicles traveling behind other vehicle maintain a safe distance at all time?

  • If vehicle breaks down, is the hazard triangle placed 20 meters on either side of the vehicle?

  • Only vehicles engineered or with correct equipment or tools used to tow other vehicles or recover equipment?

Disciplinary

  • Is the disciplinary action taken against drivers if they are found guilty of negligence after any accident or incident or where property damage has occurred?

  • Is the site had a schedule of driving infractions with the applicable sanctions?
