SafeStyle will pay certain discretions based on different criteria (excessive mileage for example). This is the form you use to claim those discretions. If it hasn't been explained ask your Trainer to do so. This form is available in excel if it makes it easier for you to keep a track of what you've claimed.

Envelope Contents Form

Whenever you send your surveys into Head Office (whether that's in person or via Special Delivery, DO NOT use an ordinary Post Box) they must have an accompanying front sheet. This sheet is then used to double check you've sent in what you say have when your surveys are received (being received is the trigger mechanism which gets you paid for your surveys).

Availiability to Provide Self Employed Services Form

In order for SafeStyle(UK) to know when you're available they need you to fill in a calendar form. These are usually emailed out 2 weeks into the month for the following month (March's would be emailed out 2 weeks into February for example). You simply put a cross through the days you're not available or a finish time on those days you want an early finish and email a scanned copy in approximately 10 days before month end. (or better still, the same day you receive it) If you're unsure how to complete one, please ask your trainer.

It is the responsibility of the Trainee to keep up to date with all the relevant FENSA & Building Regulations

As a trainee/new starter it is your responsibility to scan and email completed surveys to your Training Manager on the same day the survey is carried out, so that he may check your work. On the following morning you will receive a phone call from your Training Manager who will then run through your surveys with you and provide constructive feedback. All surveys should be posted on a daily basis, to allow Head Office to provide our manufacturing facility with a steady stream of work. Please ensure surveys are received at Head Office no later than 5 days after the completion date. After 5 days, a financial penalty will be applied.
