Equipment Inspection Log has been completed each day that it is in service.

Travel on Lifts with the platform on the ground. Do not travel with the platform raised.

Inspect that no leaks exist on any Lifts. Inspect welds and guards. Inspect that tires are in good operating condition and good air pressure. Inspect lights and alarms.

Inspect for Lifts being used as a lifting aparatus for Pipe, Duct, and Tools. Chain Falls and Duct Jacks are used for lifting purposes.

Inspect lifts for tools, equipment, and trash/debris in the bottom of the platform.

Properly dismount from the lift. Use 3 points of contact. Go out the gate and not over the rails.

100% Fall Protection at all times. Chains latched and gates locked at all times

Inspect for climbing on guardrails and climbing out of the lift.

Inspect that safety decals are in place and readable, and drive controls are labeled properly. Inspect that the manufacturer’s operations manual is in the vehicle.

Inspect that the operator is trained to operate equipment and has EAS Training Card on person.
