  • Equipment Inspection Log has been completed each day that it is in service.

  • Travel on Lifts with the platform on the ground. Do not travel with the platform raised.

  • Inspect that no leaks exist on any Lifts. Inspect welds and guards. Inspect that tires are in good operating condition and good air pressure. Inspect lights and alarms.

  • Inspect for Lifts being used as a lifting aparatus for Pipe, Duct, and Tools. Chain Falls and Duct Jacks are used for lifting purposes.

  • Inspect lifts for tools, equipment, and trash/debris in the bottom of the platform.

  • Properly dismount from the lift. Use 3 points of contact. Go out the gate and not over the rails.

  • 100% Fall Protection at all times. Chains latched and gates locked at all times

  • Inspect for climbing on guardrails and climbing out of the lift.

  • Inspect that safety decals are in place and readable, and drive controls are labeled properly. Inspect that the manufacturer’s operations manual is in the vehicle.

  • Inspect that the operator is trained to operate equipment and has EAS Training Card on person.
